Purse value Galapagos will therefore go much further up. Gilead is said to have sufficient certainty about safety profile Filgotinib with 52-week safety data. That 26-week data is not sufficient for determination of the safety profile and possible acquisition of Galapagos. I strongly doubt this for the following reasons (I believe that 26-week data is sufficient for desrisking): a) DARWIN 3 already has a treatment duration of +208 weeks for the +/- 600 RA patients included in this open label study. Approximately 2 / 3rd of the initial population respond excellently to treatment with Filgotinib. Gilead has previously indicated to be extremely satisfied with this dataset and has extended its total duration of treatment by another 3 years. Total treatment duration of DARWIN3 is fixed at 96 months = 8 years! What a confidence that patients will benefit from treatment with Filgotinb for so long. Are there MACE (DVT / PE) side effects? Minimal, or 0.1 PYE (number of cases expressed in 100 patient years). Serious infections, number of deaths within this study: Filgotinib is absolutely safe. DARWIN3 = monotherapy Filgotinib. b) AbbVie has done NDA filinig on the basis of 48-week data. The safety profile of Upadacitinib is less than that of Filgotinib on the basis of SELECT BEYOND and BALANCE Upadacitinib studies compared with those of FINCH2 and DARWIN Filgotinib studies. If AbbVie can file 48 weeks of data on the basis of Upadacitinib, Gilead / Galapagos can do 52 weekse data based on Filgotinib. c) The MANTA study is also 26 weeks duration. As already explained by Kempen extensively, here also at IEX by Rekyus, there is sufficient room to fall within the defined final values. In addition, the protocol of the study forms more of a guideline than that FDA can make a hard judgment on the basis of data. There is now a lot of thought about the way in which the admission criteria can be weakened in conversation with FDA. Nobody benefits from the fact that a superior remedy for RA, which is Filgotinib, will be kept from market authorization unnecessarily. And certainly not the FDA! Market forces for the USA market are needed; just to compete with ridiculously high pricing HUMIRA. Onno as CEO Galapagos has spoken clearly in this forum about NDA filing with FDA. Gilead does not publicly do this wisely. It needs the grace of FDA. Based on superior safety / efficiency F1 & F3 data, Gilead can join FDA with confidence. Gilead has .i. with 26-week safety data the derisking complete. That is why CEO Galapagos is already daring to pronounce the filing. Onno van de Stolpe also knows that Gilead can make a move. Purse value Galapagos will therefore go much further up.
Given that filgotinib’s performance in clinical trials has been strong, I suspect that the more significant challenge for Gilead will be commercializing the product. The market for autoimmune diseases is crowded and highly competitive, and given that upadacitinib will be first to launch and that AbbVie likely maintains a much deeper network among key players in the realm of autoimmune diseases (healthcare providers, pharmacy benefit managers, insurance companies, etc.) due to the success of its Humira franchise, it would not be unreasonable for upadacitinib to see greater market penetration, even if filgotinib establishes itself as a superior product on the safety front. Nonetheless, the sheer size of the market for autoimmune diseases ensures that both products could reach blockbuster status by the middle of the next decade. Gilead hopes to have another big winner with filgotinib. The company should announce results from a late-stage study of the drug in treating rheumatoid arthritis in the first quarter of 2019. A regulatory filing could be made later this year. If approved for rheumatoid arthritis and other key indications, filgotinib could generate peak annual sales of up to $6 billion. Gilead, on the other hand, appears to have experienced the worst for its HCV franchise. The biotech should turn the corner in 2019 with a return to revenue and earnings growth. Biktarvy is a powerhouse. Yescarta is gaining momentum. Gilead's pipeline contains multiple winners that could begin contributing to top-line growth within the next couple of years. Either of these companies could make acquisitions that change their stories. For now, though, I think Gilead is the better pick. Maar het veiligheidsprofiel van Upadacitinib blijkt nu onvoldoende. Het Advisory commitee moet er aan te pas komen bij de FDA approval van Upadacitinib en dosering 30mg Upadacitinib scoort slecht veiligheidsprofiel en zodoende stopt Abbvie hiermee. Jammer voor Abbvie die notabene zelf de kans heeft vergooid om Filgotinib in de markt te zetten.
Maxen bedankt voor een van de betere postings op IEX galapagos forum aller tijden. Deze mag dan ook niet ontbreken op draadje: onderstreept immers waarom Galapagos aandeel een waarde van 200 euro per aandeel vertegenwoordigd. Via avantiavanti 25 jan 2019 om 21:48 op het Analyst Reports 2018 draadje, van Leerink Flash 25 januari 2019: AbbVie filed the lower dose of upadacitinib for rheumatoid arthritis. AbbVie announced that the lower 15mg upadacitinib (selective JAK1 inhibitor) dose has been submitted for regulatory approval for rheumatoid arthritis, due to an internal analysis of the data from the SELECT clinical program. Though no further details were disclosed, this choice is likely due the loss of JAK1 selectivity at higher doses noted in multiple assays. The higher 30mg dose did not confer a significant efficacy benefit relative to the 15mg, and has been discarded for this indication given the known safety liabilities and potential dose-dependent increases in these toxicities. The potential upadacitinib US approval date is unknown, but the guidance is in H2. AbbVie announced that an FDA Advisory Committee has not yet been announced, but it is common for new drugs in rheumatoid arthritis to go to an Advisory Committee and the company is planning for this possibility. AbbVie’s other new immunology asset, risankizumab for psoriasis, has a FDA PDUFA date of April 25. Dit is toch wel heel belangrijk nieuws. Upadacitinib werd toch gezien als de belangrijkste concurrent van filgotinib onder de JAKs. Dit is een grote klap, en een erkenning van Abbvie dat de safety maar zozo is. Als Filgotinib haar goede veiligheidsprofiel kan handhaven bij Finch1+Finch 3, wordt het m.i. de dominante speler op de JAK markt en uiteindelijk ook, gezien de opkomende klasse van de JAKS, op de hele ontstekingsmarkt. Dat de veiligheid van filgotinib beter is, is statistisch gezien al wel langer duidelijk. Uit de in de GLPG presentaties opgenomen vergelijking is bv. duidelijk dat de hoeveelheid serious infections en herpes zoster infections bij upa meer dan 2x zo hoog ligt als bij filgo. Zie deze presentatie, pagina 19, ook bekend van de JPM presentatie en de nieuwjaarsborrel:www.glpg.com/docs/view/5c3f5764c099a-en Ik heb nu niet de data verzameld over de meest voorkomende safety problemen, zoals deze infecties en herpes zoster infections. Volgens mij is het wel zo, dat deze problemen groter zijn bij de geteste 30mg dosis dan bij de lichtere 15mg dosis. Blijkbaar zijn de verschillen zodanig, dat de verbeterde effectiviteit van de 30mg niet opweegt tegen de verslechterde safety van deze hogere dosis. Dat is niet goed voor upa. Het is uiteindelijk wel realistisch van Abbvie om dan maar zelf de 30mg dosis terug te trekken, i.p.v. dat door de FDA/EMA gedwongen worden te doen, zoals dat met baricitinib/olumiant van Lilly/Incite gebeurde. Hoe zijn de kansen dan voor goedkeuring van alleen de 15mg dosis? Ongetwijfeld groter, maar ook dit is nog niet gedaan. Gezien het feit alleen al dat Abbvie een Advisory Committee verwacht, waarin kritisch gekeken gaat worden. Behalve dat upa best wel wat safety issues rond infections heeft, is er natuurlijk de zaak van de PE/DVT's, die bij de Advisory Committee meeting van baricitinib al een belangrijke rol speelde, en mede heeft gezorgd voor de afvoer van de zwaardere dosis van 4mg, omdat de aantallen PE/DVT's opliepen bij hogere doses (als ik mij het goed herinner). Dus ik zou graag willen kijken hoe het zit met de PE/DVT gevallen bij upadacitinib. Van de Galapagos vergelijkings sheet (p19, zie hierboven) weten we al dat er bij upa een rate van 0,7 PE/DVT gevallen per 100 patient years (py) is gemeten, terwijl de background rate voor RA patienten zo tussen de 0.3 - 0.8 per 100 py is. Dus Abbvie zat daar al aan de hoge kant. Deze data kwamen van BALANCE-EXTEND, en dat is een extension van een phase 2 studie vergelijkbaar met Darwin3 voor filgotinib. Ter herinnering, Darwin3 laat een rate zien van 0.1 per 100py, dus veel lager dan de upa EN de background. Maar hoe zit het dan met de PE/DVT rate van upa in de phase 3 trials, de SELECT trials? Die info heb ik niet zo direct bij elkaar kunnen vinden, dus heb ik uit persberichten en conferentie-bijdragen zo veel mogelijk data bij elkaar gesprokkeld van de verschillende SELECT trials, en mij daarbij helemaal gefocust op de PE/DVT rates. Dus van elke trial de deelnemers per categorie (15mg of 30mg) en het aantal weken waarover gerapporteerd werd om het aantal patient years te berekenen. Dan uit de kleine lettertjes de PE/DVT gevallen gehaald, om vervolgens de rate te kunnen berekenen. Het resultaat is te zien in de tabel uit de bijlage. Disclaimer: Ik heb mijn best gedaan. Natuurlijk kunnen er foutjes zitten in de tabel. Ik heb geen updates van de SELECT trials met langere behandelduur gevonden, maar die kunnen er misschien wel zijn. Conclusies uit de tabel zijn: Gemiddeld, bij alle SELECT trials samen, is de PE/DVT rate per 100 py 1,6 voor 15 mg en 0,9 voor 30mg. De 0,9 voor de 30mg is nog iets hoger dan de eerder bekende 0,7 uit de BALANCE-EXTEND, en ligt iets boven de RA background rate van 0,8. De 1,6 voor de 15mg is nog aanmerkelijk hoger, en zal m.i. voor grote problemen voor Abbvie gaan zorgen bij deze dosis. Deze hoge rate komt ondanks het ontbreken van DVT/PE's bij 15mg van SELECT-EARLY en SELECT-NEXT. Bij SELECT-MONO en SELECT-COMPARE zorgden 1 resp. 2 gevallen bij de 15mg dosis voor verhoogde rates. Het echte probleemgeval bleek SELECT-BEYOND (de tegenhanger van Finch 2), met maar liefst 5 gevallen: 4 PE's, met bij een van die 4 patienten ook nog een DVT. (En overigens ook nog 2 PE's bij de 30mg dose). Dus opmerkelijk genoeg zitten bij SELECT trials de meeste DVT/PE gevallen bij de 15mg groep, en heeft het weglaten van de 30mg dose hiervoor dus geen zin, in tegendeel. Op basis van het totaalbeeld kan Abbvie dan ook niet gaan beweren dat het aantal DVT/PE's bij upa 15mg overeenkomt met de background rate voor RA patienten. Tenzij ze natuurlijk met allerlei statistische foefjes komen. En/of die 5 gevallen echt een statistisch toeval waren. Het lijkt me in ieder geval wel, dat als de toestemming voor de 15mg upa dose er komt, er ook hier, net als bij baricitinib, een black box waarschuwing bijkomt, A black box warning is the strictest warning put in the labeling of prescription drugs or drug products by the Food and Drug Administration (FDA) when there is reasonable evidence of an association of a serious hazard with the drug. Overigens vind ik het achteraf grappig dat Bart Filius tijdens de nieuwjaarsborrel heel duidelijk benadrukte hoe belangrijk het voor filgotinib gaat worden dat er TWEE (en niet minder) doses van filgonitib goedgekeurd moeten gaan worden, omdat voorschrijvende doctoren dan daarmee kunnen gaan spelen: Eerst de hoge dosis om een ziekte als RA eronder te brengen, dan een lagere onderhoudsdosis om hem eronder te houden. Ik dacht destijds dat hij filgo alvast aan het profileren was t.o.v. baricitinib, maar misschien wist ie toen al over het terugtrekken van de 30mg upa dosis.
Gaat het andere analisten ook opvallen dat de 30mg dosis gedropt is van upa? Ja. Gaan ze hun rekenmodellen weer voor de dag halen, waarin ze de omzet van de verschillende JAK's gaan inschatten. Ja. Gaat Filgotinib daar beter uitkomen. Ja. Gaan daarom hun koersdoelen omhoog. Ik verwacht het wel, want die komen rechtstreeks uit hun modellen rollen. Kortom: - 30mg upadacitinib ligt er verrassend nu al eruit, nog voor FDA review. - problemen voor 15mg dosis nog niet voorbij, met name bij DVT/PE - filgo profiteert Dat filgo qua safety beter scoort dan upa (en de andere JAKs) wisten we al. Galapagos: Veel ijzers in het vuur In het model van KBC Securities wordt 47% van de waardering toegewezen aan de filgotinib-franchise (in samenwerking met Gilead) en 23% aan de IPF-franchise (zonder partner). Het nieuws over de ‘fast track designation’ (i.e. proces om de ontwikkeling van geneesmiddelen voor ernstige aandoeningen die voorzien in een onvervulde medi- sche behoefte te versnellen, nvdr.) voor GLPG1972 (momenteel goed voor 1% in de som-der-delen-waardering) zou het middel naar voren kunnen duwen in de klini- sche ontwikkelingspijplijn van Galapagos. Die pijplijn onderging in 2018 enkele strategische verschuivingen, met Cystic Fibrosis dat van de prioriteitenlijst verdween, MOR106 dat werd uitbesteed aan Novartis en een nieuwe Toledo-franchise die onlangs werd gelanceerd.
