    Key points:
    1. Papillary tumor consist of 90% of all NMIBC.Urocidin has DFSrate of 35.1% at 12 months. There is no approved agent on market targetting specificallt against this form of tumor. 35.1% at 12 months is phenomenal.
    2. CIS around 10% of NMIBC , Urocidin yields DFS rate of 21% at 12 months. The only approved agent for refractory NMIBC is Valstar.
    Valstar was approved by FDA with a 18% DFS rate at only 6 months.
    3. Safety: only 2 serious adverse events (SAE) representing a rate of 1.6%, possibly related to treatment.

    I think within statistical probability, Phase IIIb should yield similar results. In that study, Urocidin is compared to Mitomycin C. In my research, there is only a very few studies (small size) with Mitomycin C used as a sole agent in BCG refractory cases. The only frequently quoted result is Mitomycin C yielded a result of "21 patients crossed over to receive Mitomycin C when BCG failed, but only four remained disease-free with a median follow-up of 64 months [17]." - 19% rate.

    My opinion: Urocidin will succeed as a viable alternative to cystectomy and will be approved by the FDA. Also, there is a very high possibility that it will be used generously in first line treatment of the NMIBC due to the excellent safety profile and tolerability of this drug. Since it is used as an off-label, the dosage price will be that of a refractory drug which is a lot higher. Thus, in the future, there is great profitability in this drug.

    Patience is the key. We just have to wait for all these to come to pass. At the mean time, we'll have to rely on Econiche (E. Coli vaccination) to drive the share price. Jennifer Shea communicated to me that Government subsidy or mandate is a distinct possibility in the coming future.
    The current share price is crazy. A few weak hands are getting too impatient. Remember, this is a stock to invest in, not a trading play. By the way, there is hardly any insider selling if you look.

    Bioniche Life Sciences Inc. (TSX: BNC), a research-based, technology-driven Canadian biopharmaceutical company, announced that the results of a large-scale commercial beef feedlot study with the Company's E. coli O157 vaccine - Econiche(TM) - have been published in this month's issue of Foodborne Pathogens and Disease (Vol. 6, Number 2, 2009), a peer-reviewed scientific journal.

    The article, entitled, "A Two-Dose Regimen of a Vaccine Against Type III Secreted Proteins Reduced Escherichia coli O157:H7 Colonization of the Terminal Rectum in Beef Cattle in Commercial Feedlots", was co-authored by David R. Smith, Rodney A. Moxley, Robert E. Peterson, Terry J. Klopfenstein, Galen E. Erickson, Gustavo Bretschneider, Emil M. Berberov, and Sharon Clowser.

    The article summarized a large-scale clinical vaccine trial of commercially fed cattle that tested the efficacy of a two-dose regimen of the Econiche vaccine against type III secreted proteins of enterohemorrhagic Escherichia coli O157:H7 on the probability to detect the same organism from terminal rectal mucosa (TRM) as a measure of gut colonization. The TRM is the most significant point of colonization by E. coli O157 in cattle. Vaccine was administered to all cattle within treated pens at arrival processing and at re-implant processing. At harvest, TRM was collected from a sample of cattle from within vaccinated and non-vaccinated pens. Seven hundred eighteen cattle were tested from within 21 pens of cattle (11 vaccinated and 10 not vaccinated) representing 3,683 cattle. E. coli O157:H7 was cultured from 68 of 718 (9.5%) TRM samples. Eleven of 382 (2.9%) vaccinated cattle and 57 of 336 (17.0%) non-vaccinated cattle were TRM culture positive. From the multilevel logistic model, vaccinated cattle were 92% less likely to be colonized with E. coli O157:H7 than non-vaccinated cattle (odds ratio (OR)=0.07, p=0.0008). This is the second published study demonstrating more than 90% effectiveness of the Bioniche vaccine against colonization.

    The researchers concluded that the two-dose vaccine regimen effectively reduced the probability for E. coli O157:H7 colonization of the TRM of commercially fed cattle at harvest. "As evidenced in this study, the number of days elapsing between arrival processing and reprocessing can be quite variable in typical feedlot systems," said Dr. David Smith, Extension Dairy/Beef Veterinarian, Veterinary and Biomedical Sciences, University of Nebraska-Lincoln. "However, we found no evidence that this affected vaccine efficacy. Therefore, feedlot managers may have some flexibility in scheduling re-vaccination without risking loss of efficacy. It is important that pre-harvest intervention strategies remain effective up to the time of harvest. Within the range of observations in this study, the protective effect of vaccine did not change as the number of days between reprocessing and harvest increased." This is consistent with the results from longitudinal vaccine clinical trials of the Company's vaccine published in 2004, 2007 and 2008.

    The Company's Econiche(TM) vaccine is the world's first vaccine developed to reduce the shedding by cattle of Escherichia coli (E. coli) O157:H7.     On-farm interventions to reduce the shedding of E. coli O157:H7 by cattle, such as simple vaccination of cattle with Econiche, have the potential to reduce food and water contamination and the consequences associated with human infection with the deadly bacteria. Clinical trials conducted with Econiche have shown a significant reduction in the amount of E. coli shed in the manure of vaccinated cows.

    Human exposure and infection with E. coli O157:H7 can result in serious health consequences, including abdominal pain and severe bloody diarrhea. In severe cases, kidney damage can occur and progress to serious complications and even death.

    Econiche is manufactured in the Bioniche production facility in Belleville, Ontario, where a $25-million expansion is taking place, supported by the Ontario and Canadian governments. Vaccine supply will be limited during this manufacturing expansion period.

    About E. coli O157:H7

    An estimated 100,000 cases of human infection with the E. coli O157:H7 organism are reported each year in North America. Two to seven per cent of those people develop haemolytic uremic syndrome (HUS), a disease characterized by kidney failure (in recent outbreaks, this percentage has risen to as high as 16%). Five percent of HUS patients die, many of them children and senior citizens, whose kidneys are more sensitive to damage.

    In addition to being infected by contaminated food or water, individuals can become infected from E. coli O157:H7 by visiting animal exhibits. Petting zoos, fairs, and agricultural exhibits provide many possible routes of transmission for E. coli. Direct animal contact is the obvious route, but contact with contaminated products (e.g., sawdust, shavings, soiled clothing or shoes) can also lead to human infection.

    About Econiche

    Econiche received international recognition in September 2007 by the Animal Pharm Industry Excellence Awards as the best new veterinary product for livestock globally.     BELLEVILLE, ON, May 5 - Bioniche Life Sciences Inc. (TSX: BNC) (ASX: BNC), a research-based, technology-driven Canadian biopharmaceutical company, today announced that the results of a controlled challenge study with the Company's Escherichia coli (E. coli) O157 cattle vaccine -Econiche™ - have been published in this month's issue of The Canadian Journal of Veterinary Research (2011;75:98-105), a peer-reviewed scientific journal.

    The article, entitled, "Vaccination with type III secreted proteins leads to decreased shedding in calves after experimental infection with Escherichia coli O157", was co-authored by Kevin J. Allen, Dragan Rogan, B. Brett Finlay, Andrew A. Potter, David J. Asper. This article summarized a controlled challenge study conducted in early 2008 at the Vaccine and Infectious Disease Organization (VIDO) - University of Saskatchewan which was the final study that led to the full Canadian license for Econiche™ from the Canadian Food Inspection Agency (CFIA) in October, 2008.

    The controlled challenge study at VIDO involved 30 placebo-treated cows and 30 vaccinated cows. The vaccinated cows each received three doses of Econiche™. Both groups were then infected with E. coli O157 and fecal shedding was monitored daily for a two-week period. These data indicate that Econiche™ vaccination had protective effects through significant reductions in both the number of animals shedding and the number of challenge organisms shed per animal. The authors of the article summarized the study by saying it provides evidence that vaccination with Econiche™ is an effective pre-harvest intervention strategy against E. coli O157.

    These data constitute part of the Company's international regulatory dossier in such jurisdictions as Australasia, Europe, Scandinavia, and South America. The Company has been actively exploring opportunities to introduce Econiche™ to markets beyond North America, where it has a full license (Canada) and a pending conditional license (U.S.).

    Econiche™ will be manufactured in the Company's newly completed Animal Health and Food Safety Vaccine Manufacturing Centre in Belleville, Ontario, currently undergoing validation and commissioning. Full production in the new Centre is expected to begin by the fall of 2011.

    Canadian vaccine offers hope for global scourge of E. coli

    Inoculating cows against deadly bacteria believed to reduce contamination in food chain
    As a deadly outbreak of a rare form of E. coli ravages Europe, the latest in a string of similar health scares over the years, scientists are in a race to come up with ways to eliminate the often deadly bacteria from the food system. Now, a made-in-Canada solution is about to hit the mass market in the form of a vaccine that would inoculate cows for the most common strain.

    Canada's Bioniche Life Sciences Inc. holds the patent to the world's first E. coli vaccine. Trademarked Econiche, the vaccine works by enabling a treated cow to produce antibodies that block proteins secreted by the dangerous strain of bacteria as it attempts to take up residence in the intestinal wall. Because the bacteria are blocked from attaching to the cell wall, they die, rather than congregate and multiply. The theory behind reducing E. coli levels in cows is that it will reduce the amount an animal "sheds" into the environment, in turn making E. coli contamination less likely in the food chain.

    Bovines carry a cocktail of E. coli strains in their intestinal tracts, none of which is harmful to the animals. Released into water, soil or onto crops via contaminated manure, the bacteria can be picked up by other animals and transferred to people. Several strains are dangerous and even fatal to humans because they produce Shiga toxins, which trigger symptoms ranging from diarrhea to untreatable kidney and liver failure. Those include O157:H7, which the new vaccine targets, and the more rare strain O104 at the centre of Germany's outbreak.

    In 2000, seven people died and hundreds more were sickened by water contaminated with O157:H7 in the small town of Walkerton, Ont., and in 1993, hundreds became ill and four died after an outbreak of the same strain in Washington state caused by contaminated beef patties at Jack in the Box restaurants. Other outbreaks of it were linked to meat at Welsh schools and fresh spinach in California.

    Food companies around the world have made huge investments in technologies that allow them to detect harmful E. coli or refine their packing processes to reduce risks. Treating cattle with a vaccine would ideally nip much of the problem at its source.

    "Canada is clearly a leader in this field," said Graeme McRae, president of Bioniche, who secured permission from the Canadian government to disseminate the vaccine back in 2008 and is waiting on licensing in the United States. It was developed by University of British Columbia microbiologist Brett Finlay to tackle E. coli O157:H7 and licensed to the company Bioniche.

    Peer-reviewed studies conducted with the vaccine have proven it can effectively reduce the amount of O157:H7 E. coli in a cow's digestive tract by 99 per cent, Mr. McRae said.

    But widespread dissemination of the vaccine has been hampered by lack of facilities to mass-produce it.

    Mr. McRae said Canada has scant production capacity for veterinary vaccines and imports most of its treatments from the United States. With $25-million in loans from the provincial and federal governments, Bioniche recently built a state-of-the-art facility that, at capacity, will churn out 500 million doses annually of the vaccine. By year's end, it will be producing 100 million doses - enough to vaccinate all of Canada's 12.5 million cattle and nearly all of the 92.6 million that U.S. officials say are in the United States.

    Two companies in the United States are also working on similar vaccines and have been granted conditional licences to produce them while continuing studies on efficacy. Canadian researchers with the federal Department of Agriculture and Agri-Food based in Guelph, Ont., are working on a means of reducing E. coli counts in animal intestines via application of antimicrobial essential oils. Early results have shown that carvacrol (from wild oregano) and cinnamaldehyde (cinnamon oil) have some effect, although trials are still under way.

    While officials have yet to pinpoint the cause of the outbreak in Germany that has killed at least 14 people and sickened more than 1,000 others, scientists have long pointed to cows as the true source of E. coli-related food illness epidemics worldwide. There are not yet any guarantees that the vaccine Mr. McRae's company is producing would be effective against other strains, although it is considering conducting tests to determine that.

    Each vaccine costs about $10 a head plus extra for the veterinarian required to supervise its delivery. The frequency required to keep E. coli counts under control depends on a given cow's situation. Beef cows should be vaccinated at six weeks of age and then twice more while at feed lots, which are notorious for spreading the bacteria because animals are packed into close quarters. Cows and bulls used for breeding in both beef and dairy should be vaccinated annually.

    Now the company needs to persuade farmers - or governments - to buy it. Some Canadian farmers have had access to the vaccine, and several thousand cows have been inoculated, but Mr. McRae suspects the cost will be a problem.

    Outbreaks like the one in Germany may convince governments around the world of the need to subsidize the vaccine to protect trading markets and public health. More research is needed to understand whether this vaccine is effective against more than one strain of E. coli. Dr. Finlay, Econiche's creator, said he doesn't know whether it would work against O104, the destructive strain sweeping Germany.

    In fact, very little is known about the bacteria, which is in only its second food-borne outbreak. Keith Warriner, a University of Guelph food scientist, applied for a grant to study the strain just before the German outbreak struck.

    "The main problem with this group of bacteria is that nobody has done any research," he said. "We had a notion of it 30 years ago and we knew [E. coli] was evolving very rapidly. We knew this was a problem. We didn't think it was going to hit us this hard this soon," he said, adding: "This is a consequence of not acting on the information we had."

    Dr. Rogan's presentation was entitled, "Pre-slaughter intervention by vaccination of cattle - new approach to reduce risk of meat contamination by foodborne pathogens, and consequently reduce incidence of human disease and economic burden". He described E. coli O157, the central role of cattle in disseminating E. coli O157 in meat, produce, water and the environment, and the rationale for the development and implementation of an efficacious pre-harvest vaccination strategy using a fully licensed vaccine (EconicheTM). He highlighted that an on-farm vaccination strategy is one of the best methods to reduce the prevalence of E. coli O157 both within its primary host - cattle - and in the general environment.

    E. coli O157 is an Enterohaemorrhagic E. coli or EHEC. EHEC strains consist of a subset of E. coli that have acquired virulence factors and are pathogenic to humans. These are shiga toxin-producing E. coli that can cause haemorrhagic colitis (HC) and haemolytic uremic syndrome (HUS) in humans.

    EconicheTM has the potential to significantly reduce the amount of E. coli O157 shed into the environment by beef and dairy cattle. E. coli O157 does not cause illness in cattle, but cattle are the primary reservoir for it. Vaccination of cattle with EconicheTM can help reduce the risk of food and waterborne contamination with E. coli O157. The Company has been actively exploring opportunities to introduce EconicheTM to markets beyond North America, where it has a full license in Canada and a pending conditional license in the U.S.

    There is a significant amount of international attention currently refocusing on the danger presented by EHEC. A new strain - O104:H4 - was identified as the causative organism of a massive outbreak in Germany in recent weeks that sickened more than 3,000 people and caused 35 deaths. "Although the Bioniche vaccine targets the O157 serotype of E. coli, other serotypes causing human infection appear to have similar attachment mechanisms to O157, and Bioniche believes that EconicheTM may provide cross-protection against other serotypes," said Dr. Dragan Rogan, Chief Veterinary Scientific Officer at Bioniche Life Sciences Inc.     The E. coli O157 organism causes no disease in cattle, but cattle are the primary reservoir for it. E. coli O157 can cause severe illness and can even be fatal when ingested by humans from contaminated meat, vegetables, other food products, or water. Human exposure and infection with E. coli O157 can result in serious health consequences, including abdominal pain and severe bloody diarrhea. In severe cases, kidney damage can occur and progress to serious complications and even death. Lingering, long-term medical conditions can persist in individuals exposed to the bacterium. These include post-infectious irritable bowel syndrome (PI-IBS), reduced kidney function, diabetes, hypertension and reactive arthritis.

    An estimated 100,000 cases of human infection with the E. coli O157 organism are reported each year in North America. U.S. outbreaks of E. coli O157 in Oklahoma, Tennessee, Virginia, Louisiana and Texas have caused 30 illnesses and one death. Another O157 outbreak in Bristol, UK caused 23 illnesses.     De intensieve veeteelt werkt nog steeds met open riolen en vervuilen het
    milieu met giftige- en als gevolg van overmatige toediening van antibiotica,
    veelal resistente bacteriën.
    Vlees, kinderboerderijen, zwemwater, grondwater en daarmee ook groenten
    kunnen besmet raken.
    Regeringen over de hele wereld intensiveren hun inspanningen om de

    Jaarlijks wordt één op de zes Amerikanen ziek door voeding.

    De concurrentie
    Pfizer koos voor een licentieovereenkomst met Epitopix voor de verkoop
    van een vaccin tegen Salmonella en het eerste vaccin tegen E.coli O157 met
    voorwaardelijke approvel in de US.
    De kosten van vaccineren tegen O157 zijn vergelijkbaar met het huidigeEconiche
    Pfizer start dit najaar met de verkoop in de US.

    De aanvraag in de US van Bioniche voor voorwaardelijke goedkeuring loopt nu 3 jaar.
    Trials voor de onvoorwaardelijke goedkeuring in de US zijn in voorbereiding.
    Er is reeds een goedgekeurde productiefaciliteit in de US.

    Bioniche heeft vaccins tegen Salmonella en Listeria in ontwikkeling.

    De bruto marges op vee van veeboeren zijn flinterdun.
    Veel veeboeren zullen daarom niet zitten te wachten op al die vaccins.

    Echter de voorkeur zal uitgaan naar gevaccineerd vlees door de
    vleesverwerkingindustrie eetgelegenheden en de consument.
    De kosten gaan voor de baat uit.
    - minder vervuiling van het milieu Grondwater, Slootwater, Zwemwater,
    Kinderboerderijen.
    - minder verspilling van voedsel o.a afgekeurd vlees en doorgedraaide
    groenten
    - meer vertrouwen in de voedselindustrie.
    - honderden miljoenen kostenbesparingen per jaar op ziekte verzuim
    ..en in de gezondheidszorg
    Het is niet alleen dat de runderen 99,5 % minder bacteriën uitscheiden.
    Echter ze produceren ook nog eens 29% minder mest.

    Door de vele besparingen die vaccineren opleveren hoeft het vlees door vaccinatie
    per saldo niet duurder te worden.
    Door de overheden gesubsidieerde vaccinatieprogramma’s behoren zeer wel tot de
    mogelijkheden.
    De Canadese overheid heeft de intentie om in 2013 voor $ 40 Mn vaccins te kopen.

    De US, Canada, Europa, Australië, NW-Zeeland, Z. Amerika en Japan zijn interessante
    targets.

    +/- 40 Mn runderen, merkvee en stalrunderen, van de in totaal 110 Mn
    runderen in Noord Amerika komen als eerste in aanmerking voor
    vaccinatie.
    Noord- en Zuid-Amerika de EU, Australië en NW-Zeeland
    zijn goed voor zo’n 300 Mn runderen.(Pag.31).

    Met een productiecapaciteit van 40 Mn doses per jaar Econiche goed voor
    13 Mn runderen en 120 Mn omzet lijkt Bioniche optimistisch gestemd.
    Mogelijk dat Econiche met 80% bruto marge vele jaren de belangrijkste bron
    van inkomsten wordt.

    Het verlies van Bioniche bedraagt momenteel zo’n 5,5 Mn per kwartaal.
    Om in een kwartaal om en nabij Break/Even te draaien zonder
    mijlpaalbetalingen zullen er +/- 750.000 runderen moeten worden ingeënt
    Misschien dat, dat mogelijk wordt in Q4 Bj 2012

    Als Bioniche, zonder orders van overheden, in 2015 in meerdere regio’s

    Urocidin FDA Fast Track nu in de afsluitende fase IIIb
    Urocidin is een minder giftig Mycobacteriële-celwand-DNA-complex +
    (MCC)en is samengesteld uit de zuivere cultuur van de Mycobaterium Phlei.
    In de onlangs afgesloten fase IIIa werd Urocidin beproefd op 129 patiënten.
    die niet verder met BCG behandeld konden worden vanwege bijwerkingen of recidieven
    en resistentie.
    In geval van terugkeer van de ziekte wordt vanwege o.a. resistentie ernstig overwogen
    de blaas te verwijderen

    Bij 32 patiënten (25%) van de 129 patiënten
    die in fase IIIa behandeld zijn met Urocidin en met BCG waren uitbehandeld, zijn na een
    jaar, geen bewijs van recidieven gevonden.
    Het respons op huidige middelen is >20%

    Vanaf december 2009 ontving Bioniche 4 mijlpaalbetaling voor fase IIIa van in totaal
    US$ 18,8 Mn.
    De bevindingen van Endo voor de afgeronde fase IIIa werden beloond met
    slechts US$ 4 Mn mijlpaalbetaling, terwijl een tussentijdse fase IIIa in november ’09
    nog met US$ 6 Mn werd beloond.
    Waaruit mogelijk af te leiden is dat de Fase IIIa niet geheel aan de hooggespannen
    heeft voldaan.

    Fase IIIb is in febr 2011 gestart voor +/- 450 nieuw geïnfecteerde- en “refractaire”
    patiënten op 120 locaties.,
    (Refractair = geen of onvoldoende respons op bestaande therapieën).
    In de afsluitende fase IIIb zal Urocidin zich moeten meten met Mitomycine C.
    Een chemo behandeling met Mitomycine C kan o.m. leiden tot beenmerg -, long- hart-
    nier- en huidaandoeningen.

    Er is nog zo’n US$ 91 Mn over van de US$110 potentiële succesbetalingen.

    Met 6 tot 21 doses per patiënt in een periode van 6 weken tot 2 jaar lijkt Urocidin met
    mogelijk 25 % volledig respons op patiënten die reeds met BCG behandeld waren een
    bruikbaar alternatief.
    De proeven waren voor patiënten met nog ingekapselde tumoren,
    zgn Carsinoma In Situ (CIS) en Papilaire tumoren ,met grote kans op progressie.

    Verreweg de meeste recidieven vinden plaats in de eertse 5 jaar.

    Mogelijk dat ook nieuwe ziektegevallen in aanmerking komen voor een behandeling
    met Urocidin.
    De jaarlijkse markt voor refractaire- en eerstelijnspatiënten in N.-Amerika is $2 Mrd groot.
    Meerdere concurrenten zullen een deel van die markt veroveren.
    25% van die markt genereert US$ 500 Mn omzet
    En dat betekent voor Bioniche US$ 112,5 Mn bruto marge = 22,5% Br..m. van de
    omzet van Endo

    Eventuele commerciële productie verwacht ik niet voor 2015

    Er zijn meerdere producten tegen blaaskanker in ontwikkeling ook in de eindfase III.
    Het Nederlandse middel tegen blaaskanker in ontwikkeling Apaziquone® (EOquin) (
    Fast Track sinds 2009) lijkt één van de meest geduchte mededingers. (Pag 30)
    EOquin is een afgeleide van Mitomycine C maar heeft niet de gevreesde bijwerkingen.

    22 juli 2009:
    “Van de met EOquin behandelde patiënten toonde 67% volledige
    respons en zij hadden weinig of geen hinder van bijwerkingen”.

    Afronding studie wordt verwacht in 2015.

    Het verkrijgen van goedkeuring van nieuwe producten in de US is geen sinecure.
    De interpretaties van onderzoeken zijn vaak controversieel en beslissingen nog
    al eens dubieus.
    Onder andere de politiek en grote biopharmabedrijven worden ervan verdacht dat
    zij niet zelden het goedkeuringstraject beïnvloeden.

    Ook een Fast Track product als Urocidin kan mogelijk nog wel een aantal beren
    op de weg naar goedkeuring moeten kunnen tackelen of ontwijken.

    De omzet kan derhalve kunnen groeien van C$ 46 Mn in Bj 2010 naar

    De kans is groot dat Endo de Human Health Divisie van Bioniche overneemt als blijkt
    dat deze afdeling haar hooggespannen verwachtingen blijkt waar te kunnen maken.

    Nogmaals de genoemde tijdschema’s en bedragen zijn geschat en
    kunnen sterk afwijken.
    Bioniche maakt haar verwachtingen zelden waar, waarmee rekening is gehouden.

    productontwikkeling te versnellen en eventueel de productiefaciliteiten uit te breiden en
    schulden ad 25 Mn op tijd af te lossen.

    Met de start van de verkoop van Econiche en met de support van de Canadese
    overheid en meerdere instellingen kunnen zich in dit boekjaar 2012 mooie momenten
    voordoen om in te stappen.

    De mogelijkheid op nieuwe teleurstellingen blijft aanwezig.

    Met mijn geschat verlies in het boekjaar 2011 van 15 Mn (1,5 Mn verlies in Bj 2010)
    sta ik voor een heel groot deel nog even aan de zijlijn.

    Kansen op een stijgende koers dit Bj 2012;

    1) start productie Econiche in de nieuwe productiefaciliteit
    2) Q3 en 4 Bj 2012 1 Mn koeien gevaccineerd met Econiche 9 Mn omzet

    tijdsschema’s in de hier omschreven scenario’s wel zo realistisch zijn ……….

    Ik kom binnenkort met een post over de laatste stand van zaken.
    in 2012 afnemende cashburn vanwege bezuinigingen en minder R&D vanwege lancering van laat fase III producten
    waarschijnlijk in de loop van 2013 duurzaam zwarte cijfers

    Hoewel ook Bioniche zelden haar verwachtingen op tijd waarmaakt.
    ben ik nu meer overtuigd dat Bioniche zal slagen en in 2015 een omzet heeft van >200 Mn.

    Resultaten Boekjaar (Bj) 2011 juli 2010 t/m juni 2011.

    3,8 Mn ontwikkelingssamenwerking.

    52% bruto Marge (1% Lagere marge vanwege lage US$ en de nieuwe laag renderende licentie-producten).

    C$ 16,8 Mn in kas
    De komende maanden wordt nog C$ 2,7 Mn tegoed ontvangen.

    De Cashburn uit Bj 2011 ad. C$ 1 Mn p/mnd zal geleidelijk verminderen door kostenbesparingen op R&D vanwege lanceringen nieuwe hoogrenderende grote- en kleine producten uit de dure onderzoeksfase III en besparingen op andere afdelingen en door prijsverhogingen. En geen of minder bouwkosten en investeringen voor equipement tbv het nieuwe productie centrum.
    Daarentegen wel extra kosten voor het opstarten van de productie.

    HUMAN HEALTH DIVISIE:
    Bioniche heeft met haar Mycobacteriële Cell Wall-DNA-complex technologoie (MCC ™) een middel tegen blaaskanker, waarbij de blaasspier nog niet is aangetast, in de afsluitende onderzoeksfase IIIb genaamd Urocidin.
    Bioniche doet deze fase in samenwerking met Endo Pharmaceuticals Inc.
    Endo neemt de ontwikkelingskosten en de wereldwijde vermarketing voor haar rekening en
    heeft Bioniche reeds $18 Mn mijlpaalbetalingen gedaan van de in totaal overeengekomen $110 Mn.
    Bioniche levert Endo fase IIIb materiaal met een goede marge.

    In de onlangs afgesloten fase IIIa werd Urocidin beproefd op129 patiënten met papillaire tumoren, en Carsinoma in situ (CIS) met grote kans op progressie.
    Deze patiënten konden vanwege, bijwerkingen of recidieven en resistentie niet verder worden behandeld met BCG.
    Papillaire tumoren zijn ingekapselde tumoren die relatief makkelijk zijn op te sporen en te verwijderen.
    Cellen met Carcinoma in situ zijn cellen met een vroege vorm van kanker. Deze zijn makkelijk te verwijderen maar zijn moeilijk op te sporen. Door vaak snelle uitbreiding op onverwachte plaatsen, de meest gevreesde vorm van blaaskanker.
    In beide gevallen is een (na)behandeling, met een direct in de blaas gespoten medicijn(chemokeur), noodzakelijk.
    35% van deze met Urocidin behandelde patiënten met alleen papillaire tumoren waren na een jaar nog ziektevrij .
    Dat gold ook voor 21% van de patiënten met een Carcinoma in Situ met en zonder
    Papillaire tumoren.
    Enkele met Urocidin behandelde patiënten hadden last van lichte tot matige hoofdpijn.
    Na behandeling met bestaande middelen keert in 80% van de gevallen binnen 5 jaar de ziekte weer terug. Men is dan vaak resistent voor het middel waarmee men eerder werd behandeld.

    Het respons op alle huidige middelen, o.a. BCG en Mitomycine C, is minder dan 20%.
    Veel producten hebben voor sommige patiënten onverdraagbare bijwerkingen.

    Vermoedelijk zal Urocidin marktconform geprijsd worden. ??>$1000??
    Mitomycine C en BCG kosten resp. >$1000 en $128 per dosis.

    De afronding Fase IIIb Urocidin wordt eind 2013 “verwacht”
    Eventuele commerciële productie wordt niet verwacht voor 2015.
    Er zullen weinig of geen berichten naar buiten komen eerder dan een jaar voor de lancering.
    Als het al ooit wordt gelanceerd.

    De concurrentie:
    Er zijn 5 producten tegen blaaskanker in Fase III
    Het Nederlandse middel tegen blaaskanker in ontwikkeling Apaziquone® (EOquin)
    (Fast Track sinds 2009) lijkt één van de meest geduchte mededingers.
    EOquin is een afgeleide van Mitomycine C.
    22 juli 2009: “Van de met EOquin behandelde patiënten toonde toen 31 van de 46 =67% patiënten volledige respons en ook zij hadden weinig of geen hinder van bijwerkingen”.
    Ook bij behandeling van de meer agressievere vorm van blaaskanker was 55% na 18 maanden nog recidivevrij
    7 febr. 2011: Top line data Fase III mogelijk in 2012
    Emda-Elektrmotorische drug delivery is in Fase III en kan zorgen voor betere opname in de blaas met als gevolg, de mogelijkheid van een lagere dosis, waardoor bijwerkingen tot een minimum wordt beperkt. Momenteel wordt de Elektrmotorische drug delivery beproefd met BCG en met Mitomycine C. De resultaten zijn veelbelovend. Respons 58% vs passieve behandeling 28% ook het aantal recidieven verminderde aanzienlijk.

    De patiënt en arts zal waarschijnlijk de voorkeur geven aan een eerste behandeling met EOquin mogelijk icm Elektromotorische drug delivery.
    Een behandeling in 2e lijn met Urocidin kan dan volgen. In dat geval staan er dus nog meerdere concurrenten in reserve. JMHO.
    Mogelijk kan Urocidin gebruikt kan worden in een eventuele cocktail een samenvoeging van de meerdere producten icm Elektrmotorische drug delivery. Maar dan zijn we weer vele, vele jaren verder. JMHO

    Er moeten nog wel een aantal beren, vòòr- en eventueel na de goedkeuring getackeld en of ontweken worden.

    Fase I van het MCC product in ontwikkeling tegen prostaatkanker is afgerond.
    Binnen nu en 2017 zullen er meerdere goede producten op de markt verschijnen.

    In het onderstaande scenario zijn voor behandeling van mensen geen mijlpaalbetalingen of andere inkomsten berekend..

    Maar wie weet.

    Het is redelijk stil rond de door Bioniche ontwikkelde Oligonucleotide ™ technologie die antikanker activiteit toonde in dieren.
