Dit heb ik me het navolgende ook wel eens afgevraagd (en ook de antwoorden zijn interessant):Vraag: BOR (op het Yahoo forum): "I'm not a biologist, but what's there to prevent TRiM platform from delivering mRNA or CRISPR payloads?"Antwoorden: Marc: That is a good question from my perspective because I think that TRIM is what distinguishes ARWR from its competition and I would include NTLA and CRSP (as well as DRNA and ALNY) in that list. CAS-9 (and the other CAS) proteins used in CRSPR editing are much larger (higher molecular weight) and have much different chemistries (they might not form stable complexes with the TRIM ligands) than the short, double stranded RNA sequences used in interfering RNA's Although the chemical structure of TRIM is proprietary (thankfully, or else we would have more competition from the likes of DRNA and ALNY than we do now), I assume that the cartoon used to depict it on the ARWR website conveys the general idea, which consists of various ligands that modify the RNA trigger to make it more stable and more compatible with the target tissue. Thus, TRIM is actually many different molecular assemblies that differ in composition depending on the target. With respect to CRSPR, the CAS protein and guide strand have to get inside the nucleus where the DNA resides, whereas in the case of SiRNA, the trigger simply has to get into the cytoplasm (i.e., CAS-RNA guide strand complex has to penetrate two, very different types of membranes, rather than just one). With respect to mRNA therapeutics, mRNA is less stable than SiRNA, which would require additional modification of the TRIM platform. Nevertheless, I have stated in numerous previous posts that I think ARWR should consider developing mRNA therapeutics that complement their SiRNA's where this is possible, For example, in theory (it may not be possible), this would enable a therapeutic to simultaneously treat both alpha-1 antitrypsin deficiency liver and lung disease by blocking the formation of Z-AAT and promoting the formation of AAT via mRNA. Hokiefun: @Marc Interesting comments. Yeah I am so wondering what does the next platform or modifications look like in their pharmaceutical strategy. These are things we have not even seen yet put into clinical combinations but you can imagine where this technology is headed if the basics are handled well in regards to their singular strategy. I would dare say market cap of 500 to trillion is very real. Say you have 25 to 50 billion in annual sales then you have these combination actions flowing through the pipelines and they start adding to prevention, massive reductions in taking meds and the outcomes are much cleaner and more healthy for the patient in the long term. I am thinking exosomes will be key carrier and rnai in its forms will probably be the main component it just appears to be a much safer, cleaner, clearer outcome right now.

But using CRISPR to treat most people with genetic disorders requires clearing an enormous hurdle: getting the molecular scissors into the body and having it slice DNA in the tissues where it’s needed. Now, in a medical first, researchers have injected a CRISPR drug into the blood of people born with a disease that causes fatal nerve and heart disease and shown that in three of them it nearly shut off production of toxic protein by their livers. Although it’s too soon to know whether the CRISPR treatment will ease the symptoms of the disease, known as transthyretin amyloidosis, the preliminary data reported today are generating excitement about what could be a one-time, lifelong treatment. “These are stunning results,” says gene editing researcher and cardiologist Kiran Musunuru of the University of Pennsylvania, who was not involved in the trial. “It exceeds all my expectations.” The work also marks a milestone for the race to develop treatments based on messenger RNA (mRNA), the protein-building instructions naturally made by cells. Synthetic mRNAs power two COVID-19 vaccines being given to millions of people to fight the coronavirus pandemic, and many companies are working on other mRNA vaccines and drugs. @Missolapola, de door Intellia bedachte oplossing gaat naar verluidt ca $ 1 miljoen dollar per behandeling kosten. Voor ziektes als AAT en Amyloidosis is dat wel te doen maar gaan verzekeraars dat ook betalen voor jicht en te hoog cholesterol, ziektes die niet 100% zeker tot een vroege dood leiden? Voor dit soort grote populaties is RNAi in combi met mRNA (zoals aangestipt op het Yahoo forum) veel effectiever. RNAi spelers hebben dat vroegtijdig onderkend door zich op grote indicaties als cardio, kanker en virusinfecties te storten. Een mRNA prikje van Pfizer/Biontech heb je al voor $ 16. Dan heb je 2 vliegen in 1 klap: je voorkomt dat de verkeerde eiwitten ontstaan en je kunstmatig gemaakte mRNA zorgt (desgewenst) voor de goede eiwitten voor een fractie van de prijs van een dna aanpassing en een veiligheidsprofiel dat vroeger voor onmogelijk was gehouden. Crispr-Cas beperkt zich voorlopig niet voor niks voorlopig (in vivo) tot gene knock-outs. Different courses, different horses.

Bedankt voor je beschouwingen, Tom. Veel gaat mijn petje te boven maar ik vind het toch ongelofelijk boeiende materie. Spectaculaire nieuwe ontwikkelingen waarmee vele ziekten kunnen verholpen worden en waar we als belegger een graantje mee van kunnen pikken. Voor mij alvast een fijne hobby!

Wederom bedankt, Tom, voor je niet aflatende speuren naar samenhang en goede interpretatie(s). Voor mij is het dikwijls moeilijk de implicaties van eea in te schatten. Zo'n bericht van Intellia lijkt meteen een kip-met-de-gouden-eieren-verhaal te worden, maar er zitten ongetwijfeld nog vele haken en ogen aan. Op Twitter schreef iemand dat gene en base editing nu zitten waar rnai 10 jaar geleden zat. In dat opzicht nog weinig te vrezen, zou je denken. De vraag is: kunnen zij die 10 jaar achterstand versneld inlopen? Of zal de FDA dat nooit toestaan...
Waar staan we met de koers volgende week? Was het pb van dit weekend goed of erg goed ? Wat moet er nu nog feitelijk volgen IVM pb's voor deze maand? Ik ben de tel dus kwijt. Toch nog veel vragen in mijn hoofd. Zonder antwoord lost het zich wel vanzelf op vanaf 15:30 in mijn onrustig koppeke.

De ziekte waar Intellia Therapeutics nu resultaten uit heeft, is Transthyretin (ATTR) Amyloidosis. Voor die ziekte heeft Alnylam vutrisiran in ontwikkeling. Afgelopen week is de NDA goedgekeurd en de PDUFA is op 14 april 2022. Vutrisiran wordt toegediend via subcutane (onderhuidse) injectie. Volgens mij wordt vutrisiran om het half jaar toegediend. Dit in tegenstelling tot Onpattro, een medicijn tegen hATTR (de erfelijke variant van ATTR). Dit wordt 80 minuten lang intraveneus toegediend, om de drie weken. Ik kan me voorstellen dat er groot gebruiksgemak is, als het kiezen is tussen een eenmalige toediening van een CRISPR 'gene editing' middel en de onhandige dosering van Onpattro. Maar wegen de risico's van gene editing op tegen een halfjaarlijkse onderhuidse injectie? Ik moet dan denken aan de gentherapieën die ontwikkeld worden voor o.a. hemofilie. Bedrijven als BioMarin Pharmaceuticals (hemofilie A) en UniQure (hemofilie B) volgen hun proefdeelnemers al meerdere jaren om de langdurige gevolgen van gentherapieën in beeld te krijgen. Ik vermoed dat dit ook zal gelden voor CRISPR-technieken. Daarom ben ik er de eerste jaren nog niet bang voor dat er serieuze concurrentie komt vanuit CRISPR hoek. Tegen die tijd is het maar de vraag of CRISPR nog wel zoveel voordelen biedt t.o.v. een halfjaarlijkse onderhuidse injectie.

Ik vond het persbericht van afgelopen zaterdag, over ARO-AAT, nu niet heel veel toevoegen aan het persbericht van 28 april. Toen kondigde Arrowhead een vooruitgang van fibrose/cirrose aan. Dit was met dezelfde 9 patiënten (twee cohorten) als waar nu sprake van was. Nu kwam er misschien iets meer 'kleur' bij de resultaten. Er wordt op YMB wel gesproken over het 'unblinden' van de andere cohorten. Ik vermoed dat dit niet gaat gebeuren. Deze resultaten zullen in de loop van 2022 duidelijk worden. Met die data in de hand zal Takeda naar de FDA toestappen en hopen dat ze een fase 3 over mogen slaan en direct voor een NDA mogen gaan. Een BTD (Breaktrough Therapy Designation) zal daarbij zeker helpen.
Hepatitis B is een belangrijke bron van kanker. Het is een voorbeeld van hoe een virus je dna beschadigt, elke knip in je dna is er één te veel. 1 op de 6 gevallen van kanker wordt veroorzaakt door virussen en bacteriën. Elke crispr-cas knip in je dna kan in mijn ogen ook weer leiden tot ongewenste mutaties. Ik vraag me af hoe die zogenaamde on-target effecten in feite getolereerd kunnen worden en hoe groot is de kans dat, als je een gen verwijdert, ondanks alle inspanningen de dubbele helix niet op de juiste manier aan elkaar verbonden wordt? mens-en-gezondheid.infonu.nl/ziekten/...
