Ultimately, I think the Big Pharma companies will drive adoption at the POC level. Pharma will probably increasingly pay for some or all of the CDx analysis. they can justify this as a cost of doing business/acquiring the scripts. The high cost of these drugs make it worthwhile for the Pharma to pay some or all of the assay cost.

BL, 1. De meeste operatoren (misschien nu allemaal) zijn opgeleid om op de 2de lijn te draaien. Om beide te laten draaien heb je ongeveer 50-66% extra operatoren nodig (als ik het me goed herinner, ik was geen specialist op de eerste lijn).Nu hebben ze wel al aanwervingen gedaan, maar ik weet niet of dit al voor alle shiften is. Maintenance en kalibratiepersoneel zou al aanwezig moeten zijn (eens de lijn is gebruikt voor productie MOETEN de onderhouden en kalibraties gebeuren, anders mag de lijn niet meer gebruikt worden). Maar dus met 2/3 van het personeel heb je een veel grotere productie. 2. Uiteindelijk wel... Om de lijn te kalibreren gebeuren véél testruns. En dit zijn allemaal rejects natuurlijk. Om kosten te drukken proberen ze wel (indien mogelijk) om zonder vloeistoffen te werken. Of water ipv de echte vloeistof. Als ik me niet vergis, was het eerste mikpunt op 85% efficiëntie en erna wordt natuurlijk nog naar verbeteringen gezocht. Om even duidelijk te maken hoe moeilijk dat is, we spreken hier over meer dan 250 processen achter mekaar (en een paar parallel), dus reken maar eens uit wat de efficiëntie van ieder proces moet zijn... De eerste lijn wordt ook nog verbeterd, maar deze lijn is veel afhankelijker van operatorfouten. Er kon soms gezien worden aan de reject rate welke operator aan welke machine stond. Deze lijn varieerde (toen ik er zat) tussen 75-85%. Nu de oude lijn gaat inderdaad niet verdwijnen, dit waren ze ook niet meteen van plan. Ze gaan de oude lijn op termijn als een R&D lijn maken eens de productioe grotendeels op lijn 2 zit. Rarezot

Als ik het goed lees zit er misschien een verbetering van 10 a 15% in? (Een efficiëntie van gemiddeld 80% op de oude lijn tegenover een potentiële efficiëntie van 85 a 90% op de nieuwe?) Of zie ik dat verkeerd? BL

Inderdaad, een stijging met 50-66% (2de lijn heeft ongeveer 2/3 van de operatoren nodig vergeleken met de eerste). Maar uw berekening is niet helemaal correct, er zijn al operatoren aangenomen voor de 2de lijn, dus die zijn al verrekend. Maar ik snap je schatting. Voor de capaciteit: 12sec/cartridge aan 100% efficiëntie (is ook verschenen in De Tijd, zonder bevestiging dat dit op 100% is). Neem een shift van 7uur (startup, cleaning, van die zaken nemen zeker een uur af), 50 werkweken (2 weken shutdown voor maintenance, kan meer zijn dit is slechts een schatting), en 85% efficiëntie brengt ons op 446250 cartridges per shift per jaar. Dit is wat ik neem als maximale capaciteit voorlopig. Miljoen is haalbaar, maar niet op 2 shiften (op 100% efficiëntie heb je 525000 cartridges per shift, maar dat is onrealistisch). Rarezot

BL Many MDx firms are actively signing CDx agreements (i.e, Guardant, Foundation unit of Roche, Fisher, Luminex, Neogenomics, etc.). From what I can see most of these competitors do not sign exclusive agreements the way BCART does. This is what makes me suspect that there is a decent milestone payment upon receipt of diagnostic registration approval. Otherwise, why would BCART lock themselves into an exclusive agreement? From the Pharma's perspective, it is an investment that gives them the first shot at a new script. For a chronic condition, this investment might be amortized over years. I think it also justifies the practice of running a specimen to look for a specific genetic mutation treated by their drug rather than waiting the 2 to 3 weeks for the NGS whole genome sequencing results. Once these NGS results make their way back to the physician, the pharma company has to hope the physician prescribes their product. There is a much better chance of capturing the script if they pay some or all of the cost of the Idylla assay and then place educational content on the platform or within the BCART secure cloud portal. I wonder if the portal also provides for records management. US HIPAA laws require that these records be maintained in a secure manner. FL

De verbetering lijkt misschien niet veel. Maar 15% minder materiaalkosten is zeker niet weinig... Waarschijnlijk zullen ze ook op zoek zijn naar nieuwe (goedkopere) onderdelen. En zoals ik al zei, ik weet niet alle details van de oude lijn en sommige assays hadden misschien een slechtere efficiëntie... Wat het lastig maakt om te vergelijken is dat de material cost voor de 2 lijnen is. Ook in 2018 waren er veel testen op de 2de lijn om deze te kalibreren en proberen te valideren. Dus die kosten waren er dan ook al. Als je in 2017 gaat kijken, dan was de efficiëntie van de eerste lijn lager, dus is die material cost niet volledig betrouwbaar als vergelijking... Rarezot

I previously said that I would not be surprised if BCART retested the lows. I think there is an equally likely scenario that the lows were an overshoot, meaning BCART may spend considerable time mulling around the share price representing the cash on the balance sheet. I can understand the 90% when it comes to car manufacturing, since they are forced to work with incredibly low margins. But if Coca Cola's efficiency drops below 90%, they will probably still be profitable... Rarezot

He was definitely speaking in terms of heavy manufacturing where lower margins are the norm. He was also making a point that with demand destruction in many industries and the need to maintain distancing that it could be hard for many manufacturers to return to profitability quickly in this environment. I tend to agree. At one point I think the assay development turnaround time may have been shorter at BCART, based on the Emergency Use Ebola cartridge released in 2016. However, this may have provided BCART a valuable lesson that it wasn't worth chasing infectious disease outbreaks. I doubt that they ever recouped the investment or got that much name recognition from the effort. That said, I suspect that ImmunExpress is pressing the FDA for expedited approval of Septicyte Rapid for use in Triaging patients. Based on news flow it seems like the FDA is approving several COVID 19 related tests per week. FL
