Anglo-Swedish drugmaker Astrazeneca said on Friday it had been subject to unannounced inspections by European antitrust regulators investigating its top-selling heartburn drug Nexium. "I can confirm that we have been the subject of inspections and that we are cooperating with the authorities," a spokeswoman for the company told Reuters. She said the inspections related "to alleged practices regarding Nexium in Europe." The European Commission said antitrust regulators had carried out raids on some firms in the pharmaceutical sector on suspicions of collusion to delay the entry of generic drugs into the market. The raids took place on Nov 30 in several EU countries, it said, but gave no detail. In October, several generic drugmakers launched cheap copies of AstraZeneca top-selling heartburn and ulcer treatment Nexium in Germany, in a move that was likely to hit its revenues in Europe's biggest market. Ze heeft reden om aan te nemen dat de bedrijven individueel of gezamenlijk de marktintroductie van een bepaald medicijn hebben proberen te vertragen. Dat is in strijd met de antitrustregels van de EU.
De Europese Commissie wil niet zeggen bij welke bedrijven en in welke lidstaten invallen zijn gedaan.

Strategic trends in the vaccine market Hedwig Kresse & Mansi Shah The vaccine market has grown rapidly in the past 5 years. Reported sales of vaccines globally from the five largest companies in the field — Sanofi Pasteur, GlaxoSmithKline, Merck & Co, Pfizer and Novartis — which account for an estimated 80% of the total market1, grew from US$7.1 billion in 2004 to over $20 billion in 2009. This strong performance has been driven by the launch and rapid uptake of novel, high-price vaccines, as well as by pandemic influenza vaccine sales. However, cost-containment, increasing competition in established areas such as influenza, a lack of innovative late-stage pipeline candidates and a strong anti-vaccination lobby are key challenges to the continuing growth of this sector. The changing vaccine market Prior to 2000, pharmaceutical and biotechnology companies generally regarded vaccines as a market with limited revenue opportunities. However, a growing awareness of the benefits of vaccination, an improved understanding of pathogen biology and the human immune system and more favourable legislation regarding vaccine injury compensation have made vaccine development more attractive. Furthermore, novel vaccine production and adjuvant technologies have aided the development of vaccines for new indications with a high unmet need. The commercial potential of vaccines has been proven by the success of novel, high-price products such as the pneumococcal conjugate vaccine Prevnar (Wyeth, now Pfizer) and the quadrivalent human papillomavirus vaccine Gardasil (Merck & Co) (Table 1). As large pharmaceutical companies are facing major revenue losses due to competition from generic versions of branded drugs, vaccines — for which the barrier to the entry of a generic version is much higher — are increasingly becoming commercially attractive. Current market The vaccine market is dominated by five companies — Sanofi Pasteur, Merck & Co, GlaxoSmithKline, Pfizer (following the 2009 acquisition of Wyeth) and Novartis (Fig. 1). Established vaccine classes such as influenza, hepatitis and paediatric vaccines account for the largest share of global vaccine sales. The outbreak of the H1N1 pandemic in 2009 led to substantial revenue increases for all major influenza vaccine players, with GlaxoSmithKline, Sanofi Pasteur and Novartis reporting combined pandemic influenza sales of $6.4 billion in 2009 and the first half of 2010. However, these high gains are likely to be short-lived, given that stockpiling of pandemic influenza vaccines has considerably subsided in 2010. On a product level, several innovative vaccines stand out as particularly successful. Prevnar and Gardasil have both achieved blockbuster status with global sales of over $1.5 billion each in 2009. Moreover, the quadrivalent conjugate meningitis vaccine Menactra (Sanofi Pasteur), the infant combination vaccine Pentacel (Sanofi Pasteur) and the rotavirus vaccine RotaTeq (Merck & Co) achieved global sales in excess of $500 million each in 2009 Entry of new pharmaceutical companies Driven by its increasing commercial potential, the vaccines sector has undergone significant merger and acquisition activity in the past 5 years, with several large pharmaceutical companies entering the field or strengthening their position (Timeline). However, their commercial outlook in this sector varies significantly. Pfizer has built a vaccine portfolio through the acquisitions of PowderMed (2006), Coley (2007) and, most importantly, Wyeth (2009). Through the deal with Wyeth, Pfizer gained access to Prevnar, an attractive vaccine pipeline and an experienced team. In 2007, AstraZeneca also entered the vaccine sector through the acquisition of MedImmune, which markets the intranasal influenza vaccine FluMist. However, although AstraZeneca gained windfall revenues owing to the 2009/10 H1N1 pandemic, FluMist's overall commercial performance has been disappointing, and we expect AstraZeneca to remain a niche company in the vaccine sector overall. Similarly, Abbott obtained an influenza vaccine portfolio through acquiring Solvay's Pharma business in 2009. However, owing to portfolio considerations, as well as a lack of competitiveness in this sector, Abbott attempted to sell its vaccine business shortly after the acquisition. Failing to attract a buyer, the company announced in September 2010 that it would cease most vaccine research and development activities and will mainly continue to produce and sell its seasonal influenza vaccine Influvac. The latest entrant is Johnson & Johnson, which agreed to fully acquire Crucell for $2.4 billion in October 2010. We believe that Crucell's attractive vaccine portfolio and pipeline will further reinforce Johnson & Johnson's already strong position in the infectious diseases sector. Overall, these developments illustrate the importance of expertise and a well-planned strategy in achieving success in the vaccine sector, which differs significantly from other sectors of the pharmaceutical industry with regard to clinical development, production, marketing and distribution. New targets and emerging markets Facing increasingly saturated markets for basic vaccines in North America and in Europe, companies are progressively looking to expand into other regions. Emerging and developing countries represent a promising commercial opportunity for vaccine makers owing to their higher disease burden and, consequently, a large unmet medical need for even basic vaccines. Local governments are also increasing their efforts to improve vaccination rates and assure supply, while growing middle classes and increasing health-care spending are creating a significant private sector opportunity. This is reflected in a multitude of deals and alliances between the major vaccine developers and domestic vaccine makers in countries such as India, China and Brazil New high-value targets also hold further opportunities for vaccine developers. With regard to prophylactic vaccines, meningitis B is widely thought of as the next near-term blockbuster opportunity, with two candidate vaccines being developed by Pfizer and Novartis in late-stage clinical trials. Developing vaccines against infections of nosocomial pathogens such as Staphylococcus aureus or Pseudomonas aeruginosa is also attractive, as increasing bacterial resistance is limiting the efficacy of current antibiotics. Vaccines against tropical diseases such as Japanese encephalitis or dengue fever offer a dual, tiered-priced market opportunity for both selected target populations (for example, travellers and army recruits) in the developed world, as well as a wider target population in endemic countries. In addition, the approval by the US Food and Drug Administration in April 2010 of the first therapeutic cancer vaccine — sipuleucel-T (Provenge; Dendreon) — for the treatment of prostate cancer has reinvigorated interest in therapeutic vaccines for a wide range of indications across several therapeutic areas, including Alzheimer's disease, substance dependence, hepatitis C and HIV. For more established prophylactic vaccine indications such as influenza or certain childhood vaccines, novel technologies such as improved adjuvants and needle-free or mucosal delivery, as well as more rapid and more flexible production methods, offer scope for product differentiation and improvement.
Future of vaccine development In the light of increasing competition, saturation of key vaccine sectors and a lack of innovative late-stage vaccine candidates, maintaining the strong growth of the past decade will be challenging. As high-revenue vaccines such as Prevnar or Gardasil reach maturity, the development of new blockbusters is hindered by scientific challenges: marketed vaccines already exist against most easy-to-address targets, leaving only complex pathogens such as hepatitis C and HIV. Furthermore, following a period of rapid introduction, reimbursement and uptake for high-price vaccines such as Prevnar, Gardasil, Menactra and RotaTeq, health-care providers in North America and in Europe have started to implement stricter cost-containment measures in the wake of the economic downturn. Looking forward, this will make it harder for pharmaceutical companies to recover the increasing development costs for more complex vaccines. Other factors limiting vaccine uptake include the widespread lack of disease awareness and a public apprehension of vaccination and new vaccine-related technologies. Owing to the prophylactic nature of most marketed and pipeline vaccines and their consequent use in healthy people, an excellent safety and tolerability profile for new candidates will remain paramount in vaccine development. Hartfalen mogelijk snel te genezen 7 december 2010, 13:40 | ANP MAASTRICHT (ANP) - Onderzoekers van het Maastricht Universitair Medisch Centrum (MUMC) hebben het gen ontdekt dat aan de basis ligt van het ontwikkelen van hartfalen, een ernstige aandoening waaraan jaarlijks duizenden voornamelijk bejaarde mensen overlijden. Dat heeft het MUMC dinsdag bekendgemaakt. De Maastrichtse wetenschappers zijn ook zeer hoopvol over een door hen ontwikkeld geneesmiddel dat het gen kan uitschakelen, waarna het hart gaat genezen. Volgens onderzoeksleider en moleculair bioloog Leon de Windt wordt al een paar jaar op experimentele basis met het geneesmiddel gewerkt. Hij verwacht dat het middel, dat zelfs effectief is bij een vergevorderd stadium van hartfalen, binnen drie jaar op de markt komt. In Nederland lijden ongeveer 180.000 mensen aan hartfalen. Er komen jaarlijks 35.000 nieuwe patiënten bij.

'Nepalese blauwhelmen bron cholera-uitbraak' 7 december 2010, 17:06 | ANP PORT-AU-PRINCE (ANP) – De cholera-uitbraak in Haïti is begonnen in het stadje Mirebalais in Centraal-Haïti waar Nepalese blauwhelmen hun kamp hebben opgeslagen. Enkele dagen na de aankomst van de Nepalese VN-soldaten werden de eerste besmettingen gemeld. Dat zeggen diplomatieke bronnen op basis van een nog niet vrijgegeven rapport van de befaamde Franse epidemioloog en cholera-expert Renaud Piarroux. Hij deed onderzoek naar de oorzaak van de epidemie op verzoek van Frankrijk en Haïti. Tot dusver ontkende de VN-missie in Haïti Minustah dat VN-militairen de bron van de epidemie zouden zijn geweest. De epidemie heeft in ruim zes weken tijd al aan meer dan tweeduizend mensen het leven gekost.

Nieuwe kans voor veelbelovende pillen Het gebeurt geregeld, tot verbijstering van patiënten. Veelbelovende medicijnen worden niet doorontwikkeld, omdat de producent er onvoldoende brood in ziet. Cinderella Therapeutics geeft ze een nieuwe kans. „De bestaande formules werken niet bij wat wij stiefkindgeneesmiddelen noemen.” In zijn statige villa in Rotterdam werkt prof. dr. Dick van Bekkum (85) gestaag voort. Omdat ook de medische wetenschap niet stilstaat. Aan de imponerende resultaten leverde hij een wezenlijk aandeel, met name in de kankergeneeskunde. „Ik heb geleefd in een tijd waarin het onderzoek een geweldige vlucht nam. Toen ik begon, leek de biochemie toonaangevend te worden. Nu is dat de moleculaire biologie. De hele genetica heb ik zien opkomen.” Na zijn pensionering belandde van Bekkum via stamcelonderzoek in de ontwikkeling van gentherapie. „Omdat we daar onvoldoende geld voor konden krijgen, hebben we noodgedwongen een onderneming opgericht. Het huidige Crucell.” Als initiator van de integrale kankercentra bevorderde Van Bekkum de snelle vertaling van nieuwe wetenschappelijke inzichten naar de klinische praktijk. „Je doet het tenslotte voor de patiënt. Alle ervaring die ik heb opgedaan, komt nu samen in Cinderella Therapeutics. Ik heb het medische veld van alle kanten gezien. Vanuit de patiënt, laboratoriumresearch, klinisch onderzoek en de commercie.” Is het eerder gebeurd dat iemand op 85-jarige leeftijd zo’n nieuw avontuur begon? Lachend: „Daar heb ik me niet in verdiept. Je moet het leuk vinden, dat is het belangrijkste.” Wordt u gedreven door onderzoeksdrift of door compassie met patiënten? „Allebei. Na mijn vertrek bij Crucell, vijf jaar geleden, ben ik als autochtone gastmedewerker aan het Leids Universitair Medisch Centrum gaan werken. Aan stamcellen, in een heel ander veld: spierziekten. Heel interessant, maar je wordt dan geconfronteerd met het probleem van veelbelovende medicijnen die de patiënt niet bereiken. Collega Vriesendorp liep daar in Amerika ook tegenaan. Daarom hebben we de formule van Cinderella Therapeutics bedacht. De bestaande formules werken niet bij wat wij stiefkindgeneesmiddelen noemen.” Wat is de reden om de ontwikkeling daarvan te staken? „Die kan van allerlei aard zijn. De nieuwe eigenaar van een farmaceutische company vindt dat het middel niet in het programma past. Soms wordt een complete divisie opgeheven. Een vaak voorkomende reden is de verwachting dat de financiële resultaten zullen tegenvallen. De farmaceutische industrie richt zich steeds meer op zogeheten ”blockbusters”: medicijnen die door heel veel mensen worden geslikt, zoals cholesterolverlagers. De belangstelling voor weinig voorkomende ziekten is gering. Terwijl we enorme vooruitgang boeken in de biomedische wetenschappen, neemt het aantal nieuwe geneesmiddelen eerder af dan toe. Bijkomend probleem is dat alle aandacht gaat naar medicijnen die we in de wésterse wereld kunnen betalen. Daar is de beweging van microsofttopman Bill Gates aan ontsproten. Die richt zich met zijn miljarden op arme landen om ook die in staat te stellen onmisbare medicijnen te kopen.” Reguliere farmaceutische bedrijven zijn onethisch bezig? „Dat zeg ik niet, maar ze zijn aan alle kanten gebonden. Door regelgeving, aandeelhouders, noem maar op. Daar hebben wij geen last van. Een winstdoelstelling hebben we niet en onze medicijnen hoeven niet geregistreerd te worden. Iedere arts mag op eigen risico een middel of behandeling voorschrijven. Is hij daar huiverig voor, dan zullen de patiënten er wat harder aan moeten trekken.” Wat is bepalend voor jullie keuze? „Een middel moet veelbelovend lijken, doet er niet toe of het voor kanker of voor psoriasis is. Het kan ook een diagnosticum zijn, waarmee je een ziekte vaststelt. Of een hulpmiddel. Een van onze projecten is een vernieuwend softwareprogramma waarmee we de resultaten van radioactieve behandeling waarschijnlijk kunnen verbeteren. De industrie heeft er geen belangstelling voor, omdat de bestaande programma’s goed verkopen. Wij gaan het nu testen en bieden het bij een positief resultaat tegen kostprijs aan.” Wie leveren de ideeën? „Patiëntenorganisaties, apothekers, uitvinders, onderzoeksgroepen… Het aantal reacties op onze website is boven verwachting. Dat zal nog toenemen, want we hebben nu ook een Engelstalige website. Een Franse vertaling komt eraan.” Hoe groot is de bereidheid om geld te steken in Cinderella Therapeutics? „Dat zal de tijd leren. Tot dusver hebben we alles uit ons startkapitaal kunnen betalen. Al onze partners werken belangeloos mee. Ons beleid is dat we pas geld vragen als we het echt nodig hebben. Dat laten we dan via onze website weten. Is het bedrag voor een bepaald project binnen, dan melden we dat ook.” Wat vindt de farmaceutische industrie ervan? „Directeur Michel Dutrée van Nefarma, de brancheorganisatie van de farmaceutische industrie in Nederland, noemt het een prima initiatief. We zijn totaal niet bedreigend voor farmaceutische bedrijven. In tegendeel, ze vinden het wel leuk als hun stiefkinderen toch in de kliniek komen.” Hoe lang blijft u dit doen? „Zolang ik het kan. Dat houden m’n medebestuursleden wel in de gaten. Begin ik te brabbelen, dan nemen ze een andere voorzitter. Mijn leeftijd vond niemand een bezwaar. Kalenderleeftijd is een achterhaald begrip. Het gaat om de functionele leeftijd.”www.cinderella-tx.org Sprookje wordt werkelijkheid Initiatiefnemers van Stichting Cinderella Therapeutics zijn prof. dr. Dick van Bekkum (85) en zijn Amerikaanse collega dr. Huib Vriesendorp (65). De non-profitorganisatie gaat afgeschreven medicijnen een nieuwe kans bieden. De stichting is enige aandeelhouder van Cinderella Therapeutics Research BV, verantwoordelijk voor de uitvoering van de plannen. Van Bekkum is voorzitter van de stichting, Vriesendorp medisch directeur van de bv. Er worden geen patenten aangevraagd. Alle informatie over onderzoek en productie komt op de website te staan en is dus voor iedereen beschikbaar. Cinderella Therapeutics werkt samen met Nederlandse en Amerikaanse onderzoekscentra en universitaire medische centra. Deelnemende specialisten testen veelbelovende medicijnen bij een kleine patiëntengroep. Het totale onderzoekstraject duurt maximaal drie jaar. Bij bewezen effectiviteit van een middel zoekt Cinderella een producent. De medicijnen worden tegen kostprijs verstrekt. De naam van de unieke farmaceutische organisatie is ontleend aan het bekende sprookje over Assepoester (originele titel: Cinderella). Ze wordt door haar stiefmoeder en stiefzussen veracht, maar wint het hart van de kroonprins. Zo moeten stiefkindgeneesmiddelen de plaats krijgen die ze verdienen. Het eerste project is de verdere ontwikkeling van een nooit geïntroduceerd medicament tegen glioblastoma multiforme: de kwaadaardigste hersentumor. Aanpassing ervan in een Amerikaans laboratorium maakte het middel waarschijnlijk nog effectiever. Het Rotterdamse Josefine Nefkens Instituut onderzoekt dat momenteel. Het medicijn zal worden getest in het Erasmus MC in Rotterdam en het Universitair Medisch Centrum Groningen.
Verslaafd aan onderzoek Prof. D. W. (Dick) van Bekkum (1925), van origine internist, koos na enkele jaren van klinisch werk voor het onderzoek. Hij groeide uit tot een autoriteit op het gebied van de nucleaire geneeskunde. Dertig jaar gaf hij leiding aan het door hem opgerichte Radiobiologisch Instituut van TNO te Rijswijk. Daarnaast was hij in Leiden hoogleraar in de experimentele transplantatiebiologie en in Rotterdam hoogleraar in de radiobiologie. In 1969 richtte de medicus-radiobioloog samen met prins Claus de Stichting Biowetenschappen en Maatschappij op, waarvan hij vele jaren voorzitter was. Eind jaren zestig was hij in Leiden en Rotterdam betrokken bij de eerste beenmergtransplantaties ter wereld. In Rotterdam richtte hij het Integraal Kanker Centrum op. In Leiden behoorde Van Bekkum tot de oprichters van het biotechnologiebedrijf Crucell, dat onlangs voor 1,7 miljard euro werd overgenomen door de Amerikaanse farmaciegigant Johnson & Johnson. Met deze commerciële activiteit wilde hij geld verwerven voor de ontwikkeling van gentherapie. Algauw namen de aandeelhouders het heft in handen. Door de overstap naar de ontwikkeling van vaccins kwam het beoogde genonderzoek niet van de grond. Met de oprichting van Cinderella Therapeutics breidde Van Bekkum zijn cv opnieuw uit. Voor geldzuchtige aandeelhouders hoeft hij nu niet bang te zijn. Alle medewerkers van Cinderella werken belangeloos. Erfelijke ziekte opgespoord in bloed moeder 9 december 2010, 13:14 | ANP HONGKONG (ANP) - Ouders kunnen in de nabije toekomst op een makkelijke manier achterhalen of hun ongeboren kind lijdt aan erfelijke ziektes. Onderzoekers uit de Verenigde Staten en Hongkong hebben een techniek ontwikkeld waardoor het DNA van een foetus via het bloed van de moeder kan worden geanalyseerd. ,,Tot dusver konden we mensen alleen testen per ziekte, maar nu kunnen we screenen op meerdere ziektes die in een bepaalde populatie voorkomen'', aldus wetenschapper Dennis Lo van de Chinese Universiteit in Hongkong. Het plasma in het bloed van de moeder blijkt het genoom (het geheel van alle genen) van het kind te bevatten. Voorheen werd gedacht dat slecht een deel van het DNA van de foetus in het bloed van de moeder zat. ,,Nu we weten dat het hele genoom van de foetus te vinden is, kunnen we zoeken naar alle erfelijke ziektes'', aldus Lo.
