Background PQ912 is an inhibitor of glutaminyl cyclase (QC), a metalloenzyme that is upregulated in the brains of Alzheimer’s disease patients. QC generates pyroglutamate Aß, a modified, pathogenic form of the peptide, by catalyzing the cyclization of an exposed glutamate at the N-terminus of Aß. The enzyme has been reported to be highly expressed in affected cortical regions in AD; the resulting pGlu-Aß has been found to be toxic, highly aggregation-prone, and a major component of amyloid plaques in humans (see Apr 2008 conference news; Morawski et al., 2014; Frost et al., 2013). PQ912 represents a small-molecule approach to reducing pGlu-Aß generation; an immunotherapy targeting this pathogenic species directly exists, as well (see LY3002813). In preclinical work, QC inhibitors have been reported to reduce amyloid pathology and improve performance in learning and memory tests in various mouse models (Apr 2008 conference news; Schilling et al., 2008). Findings The first clinical trial of PQ912 was not listed in clinicaltrials.gov; however, its results were presented at the 2013 AD/PD conference in Florence, Italy. In a single-center Phase 1 study in Switzerland, 108 healthy volunteers up to age 50 received once- or twice-daily doses ranging from 20 to 500mg of PQ912, either in liquid or pill form; 28 received placebo. The drug’s half-life in CSF was reported to be longer than in plasma, with CSF concentrations a third of those in blood. The concentration that was reached in CSF blocked QC activity. The compound's pharmacokinetic and pharmacodynamic properties reportedly were dose-proportional; for details see Mar 2013 conference news. In this trial, PQ912 appeared safe and well-tolerated up to the highest dose tested. As presented at the subsequent 2013 AAIC conference in Boston, PQ912 was also safe in an extension trial evaluating 200 to 300 mg/day in 16 elderly volunteers, as well. Many candidate drugs either activate or deactivate enzymes of the P450 detoxification complex in the liver and intestine. To assess this possibility, Probiodrug, via the CRO Covance, started a single-center trial in the United Kingdom in June 2014 that will measure inhibition of CYP3A and CYP2C19 in 36 healthy volunteers. This trial uses the benzodiazepine midazolam and the antacid omeprazole, which are metabolized by CYP3A and CYP2C19, respectively, as probes to measure whether PQ912 affects P450. From March 2015 to April 2017, the Phase 2 SAPHIR trial evaluated a 12-week course of twice-daily 800 mg PQ912 tablets to placebo. The trial enrolled 120 people with MCI or mild dementia due to AD as ascertained by CSF Aß levels at screening. It took place in seven European countries. The trial met its primary outcome of safety, with no difference in the frequency of adverse events between the treatment and placebo arms. The treatment group had more skin reactions and gastrointestinal problems than placebo, and more discontinuations. PQ912 inhibited QC activity in CSF by 92 percent. It appeared to slightly decrease pyroglutamate Aß oligomers in CSF, though CSF Aß oligomers concentrations are near the assay’s detection limit. The treatment group showed a benefit on working memory and a trend on attention, but no difference on five other neuropsychological tests. On EEG readings, the treatment group showed a reversal of AD-induced changes in theta and alpha rhythms. In CSF, the synaptic marker neurogranin and the inflammatory marker YKL40 trended downward from a known rise in AD. CSF AD biomarkers were unchanged (see Jun 2017 news). Dat maakte het in Amsterdam genoteerde Duitse biotechnologiebedrijf maandag bekend. Het bedrijf openbaarde resultaten van de eerste klinische testen in zijn soort bij patiënten met alzheimer in een vroeg stadium. Het middel bleek niet of nauwelijks veiligheidsrisico's met zich mee te brengen en had bovendien enig positief effect op de behandelde patiënten. Vervolgonderzoek zal zich onder meer richten op de dosering.
Als het maar iets effect heeft dan is dat een hele grote doorbraak , alleen al omdat veel bedrijven afhaken omdat ze er niks mee kunnen. Wel oud maar een zeer positieve melding.
De navolgende vermelding uit 2017 op Alzforum mag niet ontbreken op dit nieuwe draadje. Na een inname gedurende slechts 3 maanden waren de verwachtingen mbt verbeteringen in AD niet bepaald torenhoog. Ook peer reviews duiden op enig optimisme. New Alzheimer’s Drug Shows Safety, Hints of Efficacy in Phase 2 Quick Links 22 Jun 2017 A first-in-class Alzheimer’s therapeutic met its primary endpoint in a Phase 2a trial, according to topline results released by the drug’s sponsor, Probiodrug in Halle, Germany. The compound, PQ912, inhibits an enzyme that produces pyroglutamate Aß, a particularly sticky form of the peptide that may give rise to amyloid plaques. The main goal of the Phase 2a SAPHIR trial was to determine the safety and tolerability of a high dose of PQ912 over a three-month period. The compound appeared safe, though some participants had trouble tolerating it, with gastrointestinal and skin reactions that faded over time. The researchers did not expect to see a clinical effect in this short time frame, so they were pleasantly surprised to see brain electrical rhythms normalize in the treatment group; this measure suggests more coordinated synapse function. Participants taking PQ912 also bested controls on one of seven neuropsychology tasks. Biomarkers of synapse health and inflammation moved in the hoped-for direction in the treatment group, as well. “These preliminary results encourage us to go on to a longer study with more endpoints,” said Philip Scheltens at VU University Medical Center, Amsterdam, who was the principal investigator on the study. Other researchers agreed that the findings merit further development of the drug. “Although I am a bit concerned about the adverse events, the results obtained after such a short treatment period show that the target, the treatment strategy, and the compound are very promising,” Dieter Willbold at Forschungszentrum Jülich, Germany, wrote to Alzforum. The story around pyroGlu Aß, which forms when Aß loses its first two amino acids and the enzyme glutaminyl cyclase (QC) links up the ends of the exposed glutamate residue to make a circle, has been building for a decade. Independent studies have found that pyroGlu Aß correlates with disease severity and abounds in amyloid plaques in both AD animal models and postmortem brains. Some researchers suggest it may seed plaques, though that is not universally accepted (see May 2007 conference news; Apr 2008 conference news; Dec 2009 conference news). Oligomers of pyroGlu Aß appear particularly neurotoxic and can corrupt normal Aß42 (see Nov 2010 conference news; May 2012 news). Researchers at Probiodrug developed their QC inhibitor PQ912 to turn down production of pyroGlu Aß. Phase 1 trials on healthy young and elderly volunteers demonstrated that several different doses of the drug were safe for short periods (see Mar 2013 conference news). The maximum dose of 800 mg twice daily demonstrated no ill effects when given for 11 days, Inge Lues at Probiodrug told Alzforum. In the present Phase 2a trial, the researchers wanted find out how well a high dose would be tolerated over longer treatment periods, Lues said. The researchers recruited 120 people with biomarker evidence of AD at 21 sites in seven European countries. Participants were at early stages of the disease, with a mean MMSE of 25.5. The trial enrolled only people who were not taking acetylcholinesterase inhibitors or memantine. This was done to minimize noise in the data and better detect any signal on cognitive testing and brainwave activity, Lues noted. Scheltens said this requirement made recruitment difficult at first, but as the trial progressed, positive word of mouth allowed the researchers to attract more patients than the original goal. Participants were randomized to receive either 800 mg twice daily of PQ912 or placebo for 12 weeks.
The treatment appeared safe, with no difference in the number of adverse events or serious adverse events between the placebo and treatment arms. However, tolerability was an issue. During the first eight weeks of the study, 27 people in the treatment arm either temporarily or prematurely stopped taking the medication due to side effects, compared with six people in the placebo group. The most common problem was skin rashes or itching, which resolved when the drug was stopped. In the final month of the study, participants reported no problems. This suggests people adapt to the dosage over time, Lues said. In future trials, the researchers are considering titrating the dose up to give the body more time to adapt, she added. On the exploratory measures, the researchers saw a highly significant (p=0.002) effect on brainwave activity, as measured by EEG. In early AD, theta rhythms peak while alpha power wanes (see Moretti et al., 2009; Montez et al., 2009). In the PQ912 treatment arm, both measures became more normal, with theta power decreasing and alpha rising. The results fit with the hypothesis that pyroGlu Aß oligomers poison synapses, and that their absence allows brain function to normalize, Lues noted. The researchers are still analyzing fMRI data to find out if functional connectivity changed in the participants. In keeping with the EEG results, people on PQ912 performed better on the One Card Back test, which measures working memory, compared to their baseline performance. The placebo group remained stable on this measure. The treatment group also had a trend toward improvement on a test of attention, but no difference from placebo on five other measures. Finally, the researchers saw encouraging trends on several cerebrospinal fluid markers. The synaptic protein neurogranin rises in CSF during AD, apparently reflecting synaptic damage (see Sep 2015 news; Sep 2015 news). On drug, neurogranin edged down. The decline missed significance, but the researchers noted that if they excluded from analysis three patients who started taking donepezil during the trial, the results became significant. Meanwhile, the inflammatory marker YKL40 fell in treated patients, also just shy of significance. Besides modifying Aß, the enzyme QC also activates chemokines and spurs an inflammatory response. Thus, QC inhibitors may have a second mechanism of action that quiets the immune response, noted Hans-Ulrich Demuth from the Fraunhofer Institute for Cell Therapy & Immunology, Leipzig, Germany. Demuth helped found Probiodrug and now advises the company. The researchers examined CSF Aß and tau as well, but saw no change in these AD biomarkers. Lues noted this was not expected in such a short study. The CSF also provided signals that PQ912 was hitting its target. QC was inhibited by 92 percent in patients on drug, in agreement with Phase 1 data for this dose. Levels of pyroGlu Aß oligomers in CSF appeared to drop, but Lues noted that this data should be interpreted cautiously, since levels in the CSF are so low that they are at the limit of detection for the assay. Additional analysis from the SAPHIR trial will be presented at the 2017 Clinical Trials on Alzheimer’s Disease conference in Boston in November. The researchers plan to test several doses in future trials, and for longer time periods, Lues said. Animal studies show that inhibiting QC by 60 percent is sufficient for cognitive benefits. This level of inhibition can be achieved with a dose of 150 or 200 mg twice daily in people, she noted. “That makes us confident we can lower the dose,” Lues told Alzforum. In future trials, participants will be allowed to take acetylcholinesterase inhibitors. Probiodrug also develops an antibody against pyroGlu Aß that it plans to take to the clinic (see Nov 2015 conference news). Sinds de resultaten van Fase 2A bekend zijn geworden is de koers meer dan gehalveerd. Bovendien heeft nog niemand zich gemeld om de project te financieren en is er een uittocht geweest van aandeelhouders en bestuurders. Alle signalen staan dus op rood.

Klopt helemaal, maar toch kan ik er geen afstand van nemen. Dom waarschijnlijk, maar ik heb nog steeds hoop..
wat maak jij je toch druk ZR , als je geld er niet in zit . Ga lekker naar je eigen aandelen die ook aan het zakken zijn .

Precies Africa, zo is het. Ik heb aandelen pb omdat ik er vertrouwen in heb dat ze de ziekte met een kwart weten te reduceren. Voor de patiënten, wie heeft er niemand in de familie met alzheimer immers, en een koers die dan toch naar minimaal 100 euro gaat.
Heb ik het goed onthouden dat pb een pad bewandeld waar hooguit 10% van de concurrenten ook ongeveer op lopen!?! Bedrijven als johnsen& johnsen en Merck hoorden bij die 90%. Die 90% bedrijven stoppen er 1 voor 1 mee. Juiste samenvatting zo!?
Cholesterol is de meest verkeerd begrepen, belasterde en door het slijk gehaalde voedingsstof in de hele gezondheidswetenschap. Wat zijn we in de vijftiger jaren op het verkeerde been gezet en wat heeft de farmaceutische industrie aan deze misleiding miljarden dollars en euro’s verdiend. En wat is er een ellende ontstaan door het slikken van die onnodige, gevaarlijke pillen die niet helpen. De Alzheimer Aanpak Toen de statines enkele decennia geleden werden geïntroduceerd werd gezegd - zo lezen we in de literatuur - dat het risico op bijvoorbeeld spierpijn en bijwerkingen 0,01 tot 0,1% zou zijn. Nu weten we dat ± 2 miljoen mensen in Nederland deze pillen slikken waarvan er meer dan 20% hevige spierpijn bijwerkingen hebben ... en dit is maar één van de bijwerkingen. De spierklachten variëren van stijfheid in armen en benen tot flinke spierpijn die mensen belemmert in hun dagelijkse activiteiten. Wereldwijd worden cholesterolverlagers door meer dan 100 miljoen mensen geslikt: wat een ellende veroorzaakt deze pil! Maar nog steeds is volgens de industrie en de meeste artsen - die helaas zeggen wat de industrie ze influistert - dit medicijn nodig om je cholesterol in je bloed te verlagen, waardoor het risico op hart- en vaatziekten kleiner wordt. Het eerste klopt: het medicijn verlaagt je cholesterol in je bloed, maar het tweede klopt absoluut niet: het is nog nooit wetenschappelijk aangetoond dat het aantal gevallen van hart- en vaatziekten door statines kleiner is geworden. Het is wel aangetoond dat er miljoenen mensen ellendige bijwerkingen hebben ervaren tot aan de rolstoel aan toe door die rottige pillen. En dat terwijl je juist cholesterol nodig hebt. Dit zegt Amy Berger, auteur van De Alzheimer Aanpak, over cholesterol: “Veel van de in deze voedingsstrategie benadrukte voedingsmiddelen - eieren, roomboter, vette stukken dierlijk eiwit - hebben een hoog cholesterolgehalte. Het is belangrijk dat je je angst laat varen om deze voedingsmiddelen, die voorheen taboe waren, weer in je voedingspatroon op te nemen. Er is bijna niets dat voedzamer is voor naar voeding snakkende hersenen dan cholesterol. Dit punt kan ik niet genoeg benadrukken: cholesterol is niet de gevaarlijke stof waarvoor hij wordt uitgemaakt. Precies het tegenovergestelde zelfs. Als je ziet voor hoeveel levensbelangrijke functies in ons lichaam cholesterol nodig is - en vooral in onze hersenen - kun je je voorstellen wat er gebeurt bij een langdurig tekort, zoals kan worden teweeggebracht door een heel cholesterolarm voedingspatroon of statines of andere cholesterolverlagende medicijnen. Myeline, de beschermlaag en omhulsel van je zenuwen, bestaat voornamelijk uit cholesterol. Als er niet genoeg cholesterol in het lichaam is om dit myeline te produceren en te behouden, dan maken zenuwcellen ‘kortsluiting’, net als niet-geaarde elektrische leidingen. Het logische gevolg van zenuwcellen die elektrische impulsen niet goed afvuren en niet in staat zijn met elkaar te communiceren, is precies het soort geheugenverlies, verwarring en gedragsveranderingen die een milde cognitieve stoornis en de ziekte van Alzheimer karakteriseren.” Ga alsjeblieft dit waanzinnig belangrijke boek lezen, met name als je familie en/of vrienden hebt die lijden aan deze vreselijke hersenziekte of om jezelf te wapenen met kennis.

Het gaat om onderzoeksresultaten in een studie naar de veiligheid, tolerantie en werkzaamheid van de glutaminylcyclase-remmer PQ912 bij de ziekte van Alzheimer. De zogeheten SAPHIR-studie is de eerste klinische studie naar PQ912 bij patiënten met vroege vorm van Alzheimer. Het doel van de studie was om de maximaal getolereerde dosis, doelbezetting en behandelingsgerelateerde farmacodynamische effecten te bepalen. De resultaten van de SAPHIR-studie zijn volgens het bedrijf zeer bemoedigend en ondersteunen het potentieel van PQ912 als een nieuwe ziektemodificerende therapie. Probiodrug werkt aan een grotere proef waarin de resultaten van de studie moeten worden bevestigd.
