The more I browse competing product offerings, the more it appears that the Fisher sales team may have had conflicting interests. For instance here is a promotion for a NSCLC Gene fusion assay on Fishers Ion Torrent.

Zou het echt zo zijn dat deze verkopers een belang hadden om niet hun uiterste best te doen om hun 'eigen' product meer kans te geven? Of sterker nog: stiekem hun eigen product verkopen? Dan moet toch wel een heel gekke situatie zijn opgetreden voor BC. Ik denk dat de koers tot de Q4 resultaten tussen de 5 en 6,5 blijft. Ik vind de kans erg klein dat er op korte en middellange termijn een concurrent komt met een beter product; beter dan Biocartis kan bijna niet. Het pad naar winstgevendheid moet wel bewandeld worden. Ook met een goed product kan je het loodje leggen. Dat pad laat zich alleen zien door de verkoopvolumes (maar dat heb ik al gezegd). Daarom denk ik dat er voorlopig niets so boring is als de koers van BC. Zelfs al zou een grote belegger als J & J eruit stappen; het hoeft niets met de kwaliteit van BC van doen te hebben. MB

NEW YORK – Foundation Medicine has recalled a number of its FoundationOne CDx tests because they contained a potentially false-positive microsatellite instability (MSI)-high result, according to a recent entry in the US Food and Drug Administration's Medical Device Recalls database. The company voluntarily initiated the recall in August, informing physicians by phone that they may have received an incorrect MSI-high result in their report and asking them not to use this result to prescribe treatment. All physicians said that no treatment plan utilizing the MSI result had been applied, according to the entry, and eight reports were affected in total. According to Foundation Medicine, the issue came to light during a routine review of sequencing data and the reports were recalled within 24 hours. The eight cases were part of more than 90,000 total cases, of which more than 1,800 had true MSI-high results.

MB I think you are completely in sync with what I am tracking. I have a spreadsheet that simple divides the number of reported cartridge sales by the number of consoles. It hit a peak of 77 cartridges per console at the end of H2 2018. It has dropped down to 64 at the end of H1 2019. I'm looking for it to re-accelerate during the 2nd half 2020. The FDA clinical use approvals that are in process are critical to the inflection point in the sales trajectory i'm looking for in 2nd half 2020. I wish BCART provided more geographic information so I could track sales in a more established market like the EU vs fledgling markets like the US. As I mentioned before I often look at what Cepheid did in infectious disease a decade ago as analogous to what BCART is trying to do in oncology mDx. Cepheid had sometimes lumpy sales figures similar to BCART.. I think you are correct emphasizing partnerships. They show the breadth of potential reach of the platform. My comment about the smaller hospitals and clinics is based on the Journal of Clinical Pathology articles that include flow charts on how Idylla might be utilized in first line diagnostic applications in combination with NGS testing. I think there is a natural fit for smaller clinics, hospitals etc. That said I think there are certain product offerings that have wider appeal. For instance MSI testing by all accounts is labor and reagent intensive analysis that is made much easier by Idylla. I think both of the OncoDx products are similarly positioned for wider use. Lung cancer can have a variety of molecular drivers (hence the need for the fusion panel) a larger institution with access to faster TAT NGS testing is likely to want a more comprehensive understanding of the molecular drivers for a given patients cancer. A smaller hospital or clinic without this type of access might just follow the Pharma reps recommendation to run the quick CDx on Idylla and if it checks out prescribe the medication they are marketing. Nothing wrong with this approach, just a matter of access and resources. They will likely need the more comprehensive analysis at some point when the disease mutates. The costs of an Idylla console at US $5,500 (single) to $45,000 (for eight) is a vary reasonable investment to enable MDx diagnostics. Many of these hosiptals do not offer the service yet and are trying to figure out the best pathway to get into personalized medicine. The average single cartridge cost $200 is going to be very attractive once it is approved for full diagnostic use. Similar NGS lab tests can be 5X to 10X higher. There is potential for the user to significantly mark up the costs of the assay. This can create quite a profit incentive to utilize Idylla. Thanks again for your well researched insights. FL

Des te belangrijker is het om vol in te zetten op het omzeilen van mogelijke politieke (belanghebbende) barrières, alzeker met steun van Biocartis haar partners in die landen waar deze partners reeds gevestigd zijn. Verder moet ik er toch niet aan denken dat de koers nog tot 5 Euro terug zou kunnen zakken in afwachting van de Q4 2019 resultaten te presenteren op 5 Maart 2020.
I would not expect the trading range to fall below the historically observed range $5.40 or so. The market Cap has dropped to E$350 million, even lower than when the company IPO'd. By any measure BCART has more opportunities now than at any time since it IPO'd. The partnerships show that there is demand for fast TAT oncology MDx testing. They have just established a global distribution network and the add new product offerings every year. As I mentioned some of the ones due out in 2020 do not face significant market competition from NGS assays. Also they have added an additional manufacturing line that is cheaper to run and provides expansion potential. I believe that there are three factors that need to be resolved for BCART to take off. 1) Diagnostic testing approvals from the FDA in the US. 2) launch of unique products such as OncoDx breast at are supported by the medical community 3) Overcome inertia - many institutions have not formulated their MDx testing strategy. If and when they figure out they can offer testing cheaper and faster on Idylla it should be off to the races. FL
Je hebt volkomen gelijk: de koers van BC is nagenoeg hetzelfde als ten tijde van de IPO maar het bedrijf staat er veel beter voor dan destijds. Meestal (statistisch 75% van de gevallen) lopen de verkopen bij aanvang minder vlot en wisselvalliger dan verwacht. Daarbij is BC een diagnostica, waarbij er altijd een strijd is om terugbetaling te krijgen voor labo’s, dokters en patiënten. Dat maakt de markt, zeker voor een Europees bedrijf moeilijker te veroveren dan voor een andere biotech. Dat is inderdaad opgelost door meer FDA goedkeuringen met behulp van gerenommeerde partners. Ik vraag me af of het salesteam van BC zich ook richt op de verzekeraars/betalers. In de bedrijfspresentatie van BC zijn er weinig rekenvoorbeelden van Idylla / cartridges en geldbesparing. Dit moet verzekeraars toch interesseren. Helaas lijkt BC meer wetenschappelijk georiënteerd dan bedrijfsmatig. Dat laatste begint nu pas te komen nu ze er achter zijn dat wetenschappelijke/technologische voorsprong niet alles is en snel ingehaald kan worden. Roche gaat een klapper maken met hun test voor vaststelling van methicillin-resistent Staphylococcus aureus bacteriële kolonisatie. Dat wordt een MRSA-testgoedkeuring van de FDA. Deze test kan de beruchte ziekenhuisbacterie zeer snel en in vroeg stadium aantonen. Inderdaad zou OncoMdx een soortgelijke impact kunnen hebben maar dat wordt pas interessant in 2022? Hoelang duurt zo’n traject eigenlijk tot FDA-goedkeuring? De inertie van laboratoria medewerkers lijkt me niet onoverkomelijk en relatief makkelijk te doorbreken door het salesteam van BC. Te meer omdat Idylla modulair is opgebouwd en er oog is voor ICT-integratie. BC heeft laboratoria echt een perspectief te bieden op MDx gebied. We herhalen nu veel bij gebrek aan nieuwe input van BC. Ik richt me nu even op een Duits biotech bedrijf Biofrontera AG geheten dat AK en BCC bewezen tegengaat (EMA en FDA goedkeuringen) maar ook onnodig hard is afgestraft terwijl het net als BC de Amerikaanse markt probeert te veroveren. Dit bedrijf heeft haar zaakjes verzekeringstechnisch wel onder controle en staat wat mij betreft op de drempel van het veroveren van de wereld. MB

Je hebt gelijk: vooral de Q3 presentatie was summier. Eventuele PB's moeten wel inhoud hebben - anders worden ze doorgeprikt en dat past BC niet en worden beleggers juist argwanend. Er bestaat eigenlijk geen beter controleerbaar bedrijf als BC: aan de verkoopvolumes van haar producten kan je precies zien waar ze staat. Dat is wel wat anders als een medicijnenontwikkelaar dat in fase 2 zit, bij wijze van spreken. Bij gebrek aan PB zijn we overgelaten aan onze fantasie, aan wishful thinking , iets wat je nu juist niet moet doen. Bij gebrek aan nieuws gaan we op zoek naar tips, terwijl we weten dat er bijna nooit echt inside information gegeven wordt. (If there was easy money lying around, no one would be forcing it into your pocket.) Goed maar er is 1 trading rule waar ik me aan vasthoud: big movements take time to develop . MB There is no insurance reimbursement in the US before the FDA diagnostic use approval. Hence my harping about that topic. Therefore the labs are not getting to the potential cost savings. The 510 pathway to approval of IVD MDx products is being revamped. It was a priority of former FDA commissioner Gotlieb but he left. When GHDX was operating separately from EXAS I found that they provided more color on how central Idylla was to their expansion in the EU and in the ROW. Same can be said about Immunexpress who's website claims that Septicyte Rapid will launch on Idylla in the EU in the 1st Qtr of next year and US shortly thereafter. BCART rarely mentions this program. At one point I thought it had been dropped with the rest of the infectious disease portfolio. Flying under the radar has some advantages in that you tend to draw less fire from competitors. At some point after a registration approvals, the sales and profits will speak volumes. FL
MB How do you come up with the time frame summarized in the following statement: " OncoMdx could indeed have a similar impact, but that will only be interesting in 2022? How long does such a process actually take until FDA approval?" Based on the last GHDX conf call before the merger I have two potential questions about your post 1) The OncoDx Breast test validation was supposed to begin in the 4th quarter 2019 with an anticipated EU marking in mid 2020 and roll-out in France and Germany in H2 2020. There has been no announcement but I assume validation is underway. In the US, the sales ramp went from single digits to higher teens in a couple quarters. I assume the console and cartridge revenue will be recognized quicker than a lab test reimbursement. So I still see meaningful impact in late 2020 and definitely in 2021. If your an insurer, the cost of unnecessary chemotherapy far outstrip the OncoDx costs. 2) I don't believe that Genomic Health committed to using Idylla in the US for breast cancer. Perhaps that will change now that the EXAS merger has occurred. It is a lot easier for corporate management to eliminate all of the lab testing jobs in Redwood California when management is based in Madison Wisconsin, than when they are down the hall from the lab. GHDX indicated that OncoDx Prostate would roll out to Urology clinics on Idylla when it becomes available. I still look for late 2020 as the critical time frame. FL

FL je leest me verkeerd: tuurlijk zie en benadruk ik ook dat die FDA-keuringen er moeten komen om betalers/verzekeraars zover te krijgen dat ze gaan vergoeden. Dat zal een gamechanger zijn en het staat in al mijn stukken. Laten we elkaar goed blijven lezen! Funny thing is, I don't mind negative opinions and posts as long as I can learn something from the poster. My background is not in microbiology or genomic sciences so I appreciate those who share their knowledge. With regards to your questions, I want to refer to them by number starting top to bottom. 1) Good question - It might help estimate how staffing needs may grow over time. I'm hoping that partners with large embedded sales forces carry much of the weight of the sales ramp. BCART often says they have considerable experience/success selling into European markets after the initial placement of the console. The question that comes to my mind is when will any geographical breakdown numbers be provided so that we can gauge the value of local sales efforts and partnerships. This might provide a geographic model for how sales are going elsewhere. 2) I think BCART is still learning and won't provide much of an answer. I think I saw an ad for employment for a US based regulatory liaison type personnel. Trying to remember since I follow a number of companies. 3) My general time frame for EU launch (France and Germany mid 2020) is from memory but since it is the basis of my decision to establish a position in BCART, I'm pretty certain it is consistent with GHDX previous conference calls. Earlier this year, GHDX won insurance approval for OncoDx Breast in Germany. The FDA has already approved OncoDx Breast in the USA. The test is run in their lab in California (not IVD). I'm not aware of any existing plans, to migrate it to Idylla in the U.S. EXAS may change this if the Kevin Conroy the new CEO is looking for cost cutting synergies (I believe this is a likely bet in the future). 4) Again I think BCART is working with partners to figure out FDA approvals including PMA. I'll be surprised if they provide much color on this topic. 5) BCART has received a second round of product development funding from ImmunExpress in Feb 2019. The Immunexpress website indicates that the October financing round (Round 3) was intended to meet two goals to obtain EU and FDA 510K approvals and to provide funds to hire sales staff and launch the product next year (fist in EU then US by midyear). The question here is the financial agreement structure. I'm guessing the cartridges sales will yield a royalty payment back to BCART after reimbursing manufacturing expenses. BCART is probably providing a financial incentive for Immunexpress staff to sell or lease consoles but I'd think the bulk of any profit margin on the console would go to BCART. They have deliberately not provided many details on the financial metrics of any of their partnerships. One would assume that BCART offered deals to get initial partnerships, the incentives should tip back to BCART as more partnerships are signed and delivered. Any color on whether most deals are royalty based at rates typical of the industry would be great but I doubt you get an answer. 6) Does the technology allow transfer of educational materials on the console system? Once full marketing authorizations are received could Idylla potentially be used to deliver partner sponsored info? Probably a long way off and I doubt they will discuss it. 7) no comment Some additional questions 1) Has OncoDx Prostate reached a proof of concept point? Will BCART announce this milestone when it is achieved. This was announced for the Breast assay. 2) Can any more geographic sales breakdown information be provided? 3) How long has the Phillips intellectual property agreement been extended? Is this IP agreement exclusive worldwide or can Phillips license technology to others? 4) Currently some of the liquid biopsy tests have MAF sensitivity limits of about 5%. Is their any R&D potential to achieve similar sensitivity levels as exhibited by NGS competitors (Sysmex etc.)? I would not answer this if I was an employee of BCART, but I'm curious whether additional amplification might allow lower sensitivity levels to be achieved. Any MDx experts on the board? Vervolgens nog een aantal keren om de tijd te verdelen: ze staan me niet een uur te woord, hoogstens 10 minuten per keer. Als ik dan heldere antwoorden krijg ga ik een stukje schrijven met als advies LT in BC te gaan. Dit plaats ik op beleggerssites in NL, IEX, België, Beursduivel, etc, USA, Yahoo, Japan...? Wat zijn nog meer aangewezen beleggerssites wereldwijd?
