mmunexpress kondigde vandaag ook aan dat de SeptiCyte(R) RAPID Test voor gebruik op het Idylla(TM) platform CE-markering kreeg. Immunexpress verwacht US FDA 510(k) clearance in Q3 2020. Dit klinkt toch ook als goed nieuws?

immunexpress.com/about-septicyte/ Laup 4 Ik heb alles verkocht op 5 maart toen de outlook voor 2020 werd afgegeven. Ik heb daar toen geen ruchtbaarheid aan gegeven omdat ik in volle paniek geen olie op het vuur wou gooien. Maar ik denk dat ik wel enige uitleg verschuldigd ben omdat ik voor 5 maart nog erg positief t.o.v. Bcart stond. Ziehier mijn motivatie: De outlook was dramatisch: - verkoop in Japan nog niet gestart terwijl Bcart eerder liet uitschijnen dat dat wel al het geval was (ik heb het nog expliciet gevraagd aan Renate); - cash burn verschrikkelijk (69M) en in tegenstelling tot wat Verrelst beweert, zeggen de cijfers mij dat de cash burn volgend jaar niet sterk zal / kan afnemen, en dit terwijl ze al met een schuld van 150M zitten; - verwachtingen voor goedkeuringen van de meeste nieuwe tests alweer verschoven naar volgend jaar (wat dat betreft is de CE marking van de Septicyte test vandaag wel goed nieuws, maar dat was zowiezo verwacht voor dit jaar), - sales outlook ondermaats (en nu mogelijk nog neerwaarts bijgesteld). De sales is de enige manier om de cash burn naar beneden te krijgen. Business model: Flatlander maakt graag de vergelijking met Cepheid. Echter Cepheid verkocht zijn consoles gemiddeld 3 maal duurder dan Bcart. Hierdoor was Ceph in staat te groeien zonder al te veel cash burn. Bcart daarentegen heeft nu voor elke euro verkoop 2.72 euro kosten (cijfers van 2019)!!! Hier is enkel uit te geraken door exponentieel te groeien. Mijn model leert mij dat de cartridge verkoop ongeveer moet vertienvoudigen om break even te draaien. Dat zie ik ook in het model van de enige analist dat ik te pakken kreeg (een hele opgave!). Die analyst zag voor 2020 een cartridge verkoop van 300.000 (cijfers van september vorig jaar) en voor 2021 een cartridge verkoop van 600.000! Bcart ziet een cartridge verkoop van 228.000 in 2020 en mogelijk minder. Het is logisch dat de analist zulke hoge groeicijfers in het vooruitzicht stelde, want Bcart moet (!) wel zo snel groeien. Alleen het wil maar niet lukken. Aan het huidige tempo geraken ze nergens. Er zal wel een versnelling komen, hopelijk in 2021, maar zal dat snel genoeg zijn? De concurrentie zit ondertussen ook niet stil. Ik voorzie dat een cartridge verkoop van 1.5 a 2M cartridges nodig zal zijn voor break-even. Nog een hele weg te gaan, dus. Dat betekent ook dat mogelijk nog geinvesteerd zal moeten worden in een nieuwe productie lijn vooraleer het zover is (Bcart sprak van een capaciteit van 1M kits), tenzij men alweer dure nachtshiften gaat draaien. Nog ter zijde, veel hoop is gesteld op het feit dat de nieuwe productie lijn de productiekosten sterk zal drukken. Met deze redenering zijn 2 problemen. Ten eerste is de productiekost slechts een fractie van de totale kost (slechts 20%). Deze productiekost drukken maakt dus geen groot verschil. Ten tweede bedraagt de (raw) material en consumables momenteel 50% van de productie kost. Ik zie niet in hoe de nieuwe lijn daar verandering in kan brengen. Het basis materiaal om cartridges te maken zal waarschijnlijk onveranderd blijven, of het nu op de oude of de nieuwe lijn is. Waardering: Aan 3.8Euro per aandeel zit Bcart nu aan een P/S van 4.5 en (in de veronderstelling dat ze ooit een profit margin van 20% zullen kunnen halen) aan een P/E van 35. Uiteraard kan je nu geen echte P/E berekenen aangezien er verlies wordt geleden. Een profit margin van 20% in bv. 2025 is waarschijnlijk veel te optimistisch, dus zou de P/E wel eens eerder 70 kunnen zijn. Nu betaal je dus tussen de 35 en 70 keer de winst die er mogelijk (!) in 2025 zou kunnen komen. Geen wonder dat dit aandeel werd/word ge-short. Analisten: De analisten zijn waarschijnlijk totaal ongeloofwaardig. De analist die ik eerder vernoemde, waarvan ik de naam niet wil geven, en die 300.000 cartridges voor dit jaar voorzag, terwijl Bcart nu maar 228.000 denkt te halen, heeft zijn koersdoel gewoon behouden. Moet je mij eens uitleggen hoe dat kan. Conclusie: Verrelst wil en moet koste wat kost groeien, aan welke prijs (cash burn) ook. Daarvan is maar 1 groep mensen de pineut, dat zijn de (kleine) aandeelhouders. Op korte termijn is een aandeel natuurlijk net zoveel waard als de gek ervoor wil geven. Dit is dan ook geen betoog om mensen aan te zetten hun aandelen te verkopen of om de koers naar beneden te praten (ik maak me trouwens geen illusie dat ik dat zou kunnen, straks is dit bericht alweer onder gesneeuwd). Ik probeer enkel een antwoord te geven op je vraag waarom ik verkocht heb. Ieder beslist voor zichzelf natuurlijk, ik kan me ook vergissen. We zitten weer even in een paar dagen van "risk-on", dus Bcart kan perfect nog wat doorstijgen. Het ga je goed, Laup4. BL
Een goede analyse. Wat er echter niet klopt in dit verhaal is dat de prijs van de testen enorm omhoog kan gaan zodra er FDA- en in mindere mate EMA -goedkeuringen aan gekoppeld zijn. Het blijft wachten daarop, waarschijnlijk wel met enig uitstel. En ja enige verwatering lijkt onvermijdelijk na zoveel pech. Voor de goede orde vermeld ik nog even wat er in de koker zit. Je hebt gelijk wat betreft de prijsstijging van de testen in de U.S. na goedkeuring, maar heb jij hier concrete cijfers rond (?) en naar welke gemiddelde cartridge prijs denk je dat kan dit de globale avg cartridge price kan tillen? Ik heb in mijn model zowiezo al een hogere gemiddelde cartridge prijs gestoken (117 euro t.o.v. de 103 euro die in 2019 werd gehaald).

Biocartis heeft velen van ons al genoeg gefrustreerd. Er mag wel eens gezegd worden waar het op staat. Tuurlijk, ze hebben al wat tegenslag gehad, maar zulke dingen moet je ook incalculeren als management. Management heeft volgens mij ook de taak realistische verwachtingen bij de beleggers te scheppen, anders zijn ze hun beleggers aan het belazeren.

Ik weet hier het fijne niet van. Maar gezond boeren verstand fluistert mij in dat na FDA goedkeuring betalers/verzekeraars geadresseerd kunnen worden die veel geld kunnen besparen met onnodige behandelingen of met gerichte behandeling of ingekorte oncologische behandeling. Het kan niet anders zijn dat de testen dan ook behoorlijk gaan stijgen omdat BCART dan een gedeelte - terecht - opeist. Als je dan de prijs van concurrerende testen vergelijkt zie je dat er behoorlijke marge is voor BCART om daar goed gebruik van te maken. Dit is iets wat Renate mij ook bevestigd heeft. De VS blijven toch de belangrijkste markt hierom. Ik verwacht nog steeds - op basis van de technologische voorsprong van BCART - dat darm en lonkkanker testen een succes gaan worden na FDA-goedkeuring. Als die niet binnenkomen, eerste half jaar 2021 neem ik mijn verlies.
There are a lot of points to discuss this morning. Please forgive me for doing it in a single post. First let me start with the BCART downplaying of the guidance that was provided on March 5. As I wrote previously, I was surprised BCART even offered guidance given that almost all companies were not giving guidance due to the uncertainties associated with Covid 19. The fact that they are hedging by adding the words "may potentially impact" into the guidance is understandable since the entire world has changed. My positive take away is that the disruption is due to temporary clogging of the sales channels and not disruption of the supply chain. BCART is still producing, the problem is that almost everyone in the health provider world has their attention trained on Covid 19 and not oncology MDx research. The positive thing is that Septicyte rapid gives them a product that is applicable to the current pandemic. As such, I think the sales channels (hospitals, clinics in the EU) will be more receptive to sales attempts. Please excuse the following blunt/morbid discussion, From my reading about COVID 19 I understand that a percentage of the deaths are due to respiratory distress due to fluid in the lungs. Another percentage of the deaths are due to systemic organ failure due to sepsis. Respiratory failure can be treated with a ventilator assuming one is available. Multi organ failure due to sepsis caused by an infection with no treatment is an acute death sentence. Septicyte rapid can be used to enable Drs. to determine how to allocate care to patients with the potential to survive. BCART and Immunexpress have not released the financial terms of their agreement. My original assumption was that BCART would receive a typical royalty as a medical device partner (cover their production costs and 10% of profits is typical in the pharmacy business). Based on today's announcement, I believe the agreement will be more favorable to BCART (receive a higher percentage of the profits). As shown below, BCART is taking the lead in the EU on the commercialization and that will result in a higher royalty than my base assumption (maybe up to 50%). "Biocartis and Immunexpress are strengthening their existing partnership with a co-commercialization agreement in which Biocartis will lead commercialization in Europe as the exclusive distributor of the SeptiCyte®" In a practical way it could work no other way since travel is now restricted so an dedicated sales force already in place with contacts at the POCs is critical to success of the launch. This provides the BCART sales team in Europe a product that is applicable to the current emergency. Now to address some of BLs comments. I respect that your decision to exit BCART is the right one for you. However, in our past exchanges I have often cautioned that your models are based on extrapolation of the current RUO sales into the future. I believe a quantum leap occurs when a product designed for the Point of Care (POC) market is no longer blocked from this market (i.e., selling only into the RUO facilities). As I have elaborated on in prior posts, the Idylla console costs are currently about 1/10 of the cost of a Fisher or Illumina product that is being marketed for the POC users. Upon the approval of major assays (MSI, Gene Fusion, etc.), I believe the console price will be increased such that these sales will also generate profits. The Cepheid example that I often quote is to demonstrate how well a POC cartridge based MDx system is received by the market due to the quick results and ease of use. Yes, Cepheid had the advantage of higher console/equipment placement profit margins early during its launch. Gene Expert had the luxury of being able to ride the tide of quick approvals of its content for diagnostic use. Diseases such as anthrax, H1N1 and tuberculosis resulted in scares that lead to quick diagnostic use approvals. This enabled Cepheid to charge more for the console. Cancer when viewed across the entire population is more of a chronic rather than acute health concern). As such, Idylla assay content is subject to a longer review. Also, a significant portion of Cepheid's sales were the high throughput infinity units that could analyze something on the order of 50 tests at a time. This increased the average profit margins on the front end instrument placement. Currently, the RUO game is to place a console at tier 1 research facility at cost in hopes that the user will write a research paper that is favorable in order to help with future sales. This is happening but it is not a profitable model. I would not base my future sales/profits assumptions that this continues until 2024 when the company tells you it will be shifting to the POC model in the next year well before that time frame. However, it is a necessary step for Idylla assays to gain POC diagnostic approval from FDA or Japan and China's regulatory authorities. I think Asia may emerge as a significant market before the US. The US response to COVID 19 is a patchwork of actions at the state and city levels. Unfortunately, without travel restrictions I fear that the outbreak will persist longer and shift geographically across the US and reoccur numerous times. This may make it a difficult sales environment until a vaccine is available. Asia is emerging quicker from their outbreaks. The delay in significant sales in Japan following the November approval is understandable. Nichirei is a conglomerate not a research institution, it is likely to allocate resources based on the immediate potential to monetize its investment. They most likely have most of the consoles they need for preparing the registration applications. The sales ramp occurs when they receive diagnostic use authorization and can be reimbursed by insurance for the tests that are run. The 2000 console number floated in a previous press release likely assumes widespread assay content approved for diagnostic use. If I were BCART, I would slow the US expansion hiring until the US gets a better handle on the pandemic. Just in recap, I would not extrapolate the current RUO sales to 2024. Diagnostic use approvals will enable BCART to leverage the much larger sales forces of its partners (BMS, AZN, Amgen, EXAS etc.) such that the cost of sales will decrease at the same time that the sales revenues increase. This is a formula for the path to profitability that is not reflected by your financial modeling. Regards FL

But you must agree sticking a valuation on a company which does not provide a number of key figures ( and they are numberous ) is pretty difficult . So far you can not say that managment although they have a great product gave evidence of great leadership and i am talking before corona
FL I will not reply to all your comments about Covid-19. As I pointed out, it may well be that the Sepsis test is beneficial for sales, but despite this possibility, Bcart still warns they may not reach their targets for 2020. Concerning your comments on me extrapolating RUO sales to 2024. The point is I don't, because I put in an exponential growth (cartridge sales times 10) by 2024 and even then they barely break even. Now it is very well possible that the avg sale price per cartridge goes above the 117E as MB pointed out, but the question is how much. Also, when Bcart leverages the larger sales forces of its partners, the question is what that will do to Bcart's profit margins. I cannot imagine the margins will be as high as when they do sales themselves, otherwise what would be the point of doing the selling themselves in Europe? This margin contraction may well offset the gain of higher cartridge prices in the U.S. As you see there are a lot of "ifs". The only thing we can be certain of is that costs won't go down in the foreseeable future. Anyway, I would love to see your financial model. It may explain your optimism.
COVID 19 and Sepsis The article would be of a little more use if they would provide a survival percentage for patients developing COVID 19 patients that develop sepsis. Gijpie I agree that BCART could make it easier on the individual investor by providing geographic breakdown that allowed us to assess the growth rates for RUO vs diagnostic approval. Further granularity of discussing countries with insurance reimbursement codes would be even more valuable. Anecdotal statements that they see more customers moving Idylla to 1st line diagnostic use are helpful but not very quantitative. Discussing specific percentages and the types of organizations making such moves would be more useful. As I have said all along, BCART is a speculative stock and for most people should probably not comprise a major portion of their portfolio. I love the fact that the user reviews and the pathology reviews are great. I also love the fact that there is no other similar fast TAT, ease of use MDx oncology product on the market. I love the size and strength of BCARTs partners that have recognized this value. I hate the the process necessary to access the POC diagnostic market. I also hate the amount of time necessary to go from proof of concept to validation. Sometimes I try to reassure myself that these regulatory hurdles will becomes a moat that makes it difficult for competitors to launch into the same POC market. However, this is the thing I'm least certain about. FL

This margin contraction may well offset the gain of higher cartridge prices in the U.S." I do not see much if any margin contraction due to partners carrying much of the sales burden. The sales incentive for the partners to promote the placement of Idylla systems is huge. In the CDx environment, it is all about capturing the script before your competitor (where one exists). Having a CDx partner like Idylla that turns the assay around in hours (weeks faster than your competitors partner) is a huge advantage to the Pharma partner in capturing the script. Partners will pay a premium for this advantage. I also think your models do not include much in the way of collaborative revenues. We will have a better idea what the milestone payments are for successful registration when the first of the CDx's are approved for diagnostic use (hopefully within the next 12 months if any timeline still holds in this CV world). Granted these collaborative revenues are not reoccurring revenue but they are not insignificant when several of them are in the works. I'll be frank that part of my projections are based on significant worldwide expansion of a killer ap such as Oncotype Dx Breast assuming a typical royalty arrangement. I do get concerned when there are delays to these assumptions coming to fruition. However, there are many shots on goal with BCART. If it is not Oncotype Dx then quite possibly it is traction in a large population center like China. Like MB, I also do not believe that BCART will maintain pricing that in some cases is half of their competitors pricing and well below the insurance reimbursement rates (see my prior posts) after full diagnostic authorization. BCART will likely increase prices to achieve margins more typical of the industry (i.e., gross margins in the 50% range and net margins around 10%). There so many moving parts here that you could drive a truck through your model output. For instance, how much of the future content will be proprietary (MSI, KRAS, NRAS, Fusion, etc. vs how much will be partner sponsored content (Septicyte, OncoType Dx) launched on the idylla platform? What will the royalty payments be from these partners? The Septicyte rapid launch should start to provide some insights into this pricing and royalties (assuming BCART shares some of these figures). FL Driving a truck through my model can work both ways. I may even have been too optimistic in my calculations. And rest assured, the collaboration revenue did not escape my attention either (I even asked Renate for more specifics on that matter, but the answer was as to be expected no answer). MB and yourself bring up a few valid points, but the big question will be how long it will take Bcart to execute while the cash is on fire. They have not been very successful in doing so, so far, and as it now stands we are again looking at a lost year. Investing in Bcart has become a matter of faith. And my faith is the one thing that Bcart management has managed to destroy. It is nice to hear they did not yet destroy yours. BL
BL We are in agreement on the transparency issue. Almost all companies are in a similar situation regarding the potential for this to be a lost year. I certainly understand your skepticism since there have not been a lot events that instill confidence. I largely made up my mind early, that I would give BCART enough time to get the product to the point that multiple assays entered the POC diagnostic environment. I limited my holdings to a speculative allocation. The septicyte assay is one of the few tests that has seemingly launched in a timely manner (relative to what the partner Immunexpress had published as guidance). You are correct that they simply rely on too much of a "trust us" component. But there are few similar firms that have a product with unique distinct advantages that have capacity to sell at a significant discount to competitors in a market that grows at a rate of more than 20% a year. But the waiting for them to achieve an economy of scale is a killer. As I have said before, you could go back more than 2 years on this board and read similarly optimistic posts. Keep reading and contributing, BCART may eventually provide the information that increases the visibility to the point we all desire. Regards FL

There is a short window near the onset of sepsis that anti-inflammatory drugs may slow the spread of organ failure. The 90 min TAT of Idylla is not the 15 to 20 mins the article is looking for but it is a significant improvement. If resources are tight, septicyte rapid will likely be a tool that can be used in a triage capacity to identify patients with the best chance of long term survival. FL
