(zie bijlage pdf) UNIQURE, N.V. August 21, 2014 INVESTMENT THESIS We rate QURE Outperform with a $28/share price target. QURE is a leader in the field of gene therapy and has developed the first and currently the only gene therapy product to receive regulatory approval in the Western World. Gene therapy offers the prospect of long-term and potentially curative benefit to patients. QURE's Glybera has been EMA-approved for treatment of patients with lipoprotein lipase deficiency (LPLD), a ultra-rare metabolic disease. Glybera is partnered with Chiesi Farmaceutici in Europe and QURE retains full commercial rights in the United States. QURE is developing a pipeline of additional gene therapies through multiple collaborations that are designed to accelerate the development and commercialization of these programs. QURE's gene therapies are delivered through an engineered non-replicating version of the adeno-associated virus (AAV) and therapies are being developed using an innovative, modular technology platform, which consists of a suite of components that may be applied to multiple gene therapies and includes QURE's proprietary cost effective commercial scale manufacturing process. We believe QURE shares are poised to appreciate primarily driven by progress of the Glybera launch in Europe, clinical progress toward approval in the U.S., and by advances of pipeline candidates targeting significantly larger indications than Glybera, such as Hemophilia-A and -B, congestive heart failure, Sanfilippo B, Acute Intermittent Porphyria, and Parkinson's Disease, among others. QURE has multiple potential meaningful catalysts over the next two years. VALUATION We estimate a $28 fair value for QURE shares in one year, based on a discounted multiple of sales analysis. We attribute $5/share to Glybera EU royalties, $4/share to the Glybera US franchise and assume a 70% probability of success, $14 to AMO-060 in Hemophilia B, assuming a 30% probability of success, and the rest to net cash. We use a 12% WACC as the discount rate, similar to other small-cap biotech stocks in our coverage since our revenue assumptions are already probability-of-success weighted. We model 2025 US Glybera sales of 46M EURO, 2025 Glybera EU royalties of 22M EUROs, 2025 US AMF-060 sales of 233M EUROs, and 2025 EU AMT-060 royalties of 82M EUROs (all before probability adjustment). RISKS TO VALUATION An investment in QURE is fundamentally a high-risk, high-reward investment, in our opinion. For risks associated with LPLD in EU, a poorer-than-expected launch, reimbursement, and inability to identify patients despite approval could harm the stock. In the US, Glybera has additional clinical, regulatory risks. On the hemophilia front, competition from Longer Acting Factors (e.g., BIIB), RNAi (e.g., ALNY) and other Gene Therapy companies could deem QURE’s program as risky. Clinical and regulatory risks remain with QURE’s early hemophilia program. Systemic impact from other Gene Therapy failure such as a new case of leukemia with competitor initiative could pose significant risk. Finally, financing risks also remain and we don't expect the company to be cashflow positive for several years.

The company reported a slight delay with regard to the regulatory process for Glybera (LPLD) in the US. More time than initially expected is needed to align EU post-approval trial requirements with US pivotal trial design. We are now projecting a potential US Glybera launch in early 2019 vs. mid-2018 previously. Other pipeline programs remain on track. QURE ended 2Q14 with €72.1M in cash and equivalents. We continue to believe QURE shares are poised to appreciate primarily driven by progress of the Glybera launch in Europe, clinical progress toward approval in the U.S., and by advances of pipeline candidates targeting significantly larger indications than does Glybera, such as Hemophilia-A and -B, and potentially congestive heart failure, among others. We reiterate our OP rating and $28 price target. • Six months delay in Glybera US regulatory progress expected. This delay is in part the result of longer-than-expected EMA approval timelines due to required changes in the trial protocol in support of the US filing strategy, resulting in a planned Phase 4 trial start now in mid-2015, and in part the result of limited product supply due to a Glybera-specific batch release assay being out of specification. Recall, the FDA previously indicated in a pre-IND meeting in 2H13 that the company can use the data from the required EU post-approval trial and patient registry for US approval, but with changes to an endpoint measure definition. QURE is currently amending the trial protocols so that they could also serve as a single clinical program with a design that addresses the FDA's requirements for US approval requiring a change to EMA post-approval requirements. Another FDA meeting is planned for November 2014 as well. Trials are now expected to initiate in mid-2015, and mgmt has suggested that it would take ~1 year to enroll and another 2 years for follow-up. We are now projecting a potential US Glybera launch in early 2019 (vs. mid-2018 previously). With regard to manufacturing, mgmt noted that the company expects to be able to meet 2015 EU commercial demand with current stock and from future production runs commencing later in 2014, assuming higher manufacturing standards are successfully implemented. The European launch of Glybera is still expected in 4Q14/1Q15. • Other pipeline programs remain on track. Initiation of the Phase 1/2 trial for hemophilia B is expected in 2H14. Collaborator-sponsored programs for acute intermittent porphyria and Sanfilippo B are on track with data expected in 2H14 and 2015, respectively. • QURE ended 2Q14 with €72.1M in cash and equivalents. We update our model to reflect 2Q14 results. Een verschil van 250 (!) procent. Ik vind het wel een hele grote afwijking hoor. Ook de vertraging van Glybera in de US wordt wel erg licht opgevat. Het zou leuk zijn als ze de eerste met goedkeuring voor een gentherapie in de US worden, maar of dat gaat lukken valt te bezien.

Ja heel leuk, maar met de eisen die de FDA stelt zijn er mogelijk geen patienten meer over. Het is een therapie die met 1 behandeling levenslang werkt. De patienten staan denk ik in de rij voor de clinical trials. Zeker nu de goedkeuring in de EU er is. Gratis van je probleem af. En voor deze patienten is iedere dag er eentje, want het is een behoorlijke klus om pijnvrij te leven met LPLD. (een worteltje bevat nl al teveel vet). UniQure doet dit dus echt om een bevestiging te krijgen dat het werkt; rijk gaan ze er niet van worden. Gr Frank

Helemaal mee eens professor. Betaald worden voor onderzoeksdata is natuurlijk prachtig.

Reuters By Deena Beasley LOS ANGELES, Feb 19 (Reuters) - As U.S. drugmakers face growing resistance to the high price of cutting-edge treatments, a handful of companies are working on a new payment model that rewards them for the long-term performance of their medicines. The effort, industry executives told Reuters, is being led by firms developing so-called gene therapies, which aim to cure inherited diseases like hemophilia by "fixing" the single faulty gene responsible for the disorder. They include BioMarin Pharmaceutical Inc in San Rafael, California, and Sangamo BioSciences Inc in Richmond, California. If these new hemophilia drugs and others like them succeed, a one-time infusion could replace the need for frequent, life-long injections of blood clotting proteins that can cost up to $300,000 a year for a single patient. These existing treatments, including Pfizer Inc's Xyntha and Baxter International's Advate, are expected to command annual sales of more than $11 billion by 2016. Drugmakers contend that a one-time cure, even at a price of more than $1 million, would save money over the long term. But there are concerns that health insurers will balk at covering that kind of upfront cost. "The place that we are moving toward is more of a pay-for-performance type of strategy," said Edward Lanphier, Sangamo's chief executive officer. Under this model, the price would be amortized over a period of time and contingent on proof that the treatment is effective and safe. The annuity-like payments would be stopped if medical testing, such as the level of clotting protein measured in a patient's blood sample, showed that the therapy was not working. Many barriers remain to implementing such a model, and the drugs for which it is being considered may not reach the market for several more years, if at all. Since Americans often switch health insurers, contracts - or even legislation - would be needed to require payers to pick up the ongoing tab for patients who change their coverage. But the interest in new payment models reflects the healthcare industry's intention to find new ways to bolster profits as insurers push back against drug prices. Some backers of the new model say the payment streams could eventually be packaged and sold to investors, as happens now with securities backed by financial assets like credit card receivables. "Our approach to this important funding-benefit question is to establish a financing vehicle where the relatively large upfront cost is amortized in conjunction with the benefit of these innovative, curative or preventative therapies," said Michael Meyers, managing director and head of investment banking at T.R. Winston & Company, which is discussing such funding models with pharmaceutical and biotechnology companies. Dr. Troyen Brennan, chief medical officer at CVS Health Corp , the second largest U.S. pharmacy benefit manager, is supportive of such efforts. "Some sort of model where there was a partial payment over time could work well," Brennan said in an interview. THE PRICE OF PROGRESS Gene therapy has been a target of Big Pharma for more than 20 years, but research has been dogged by a series of disappointments and safety concerns. More recent scientific advances have paved the way for the potentially life-changing treatments. No gene therapies have been approved in the United States, but Europe approved its first gene therapy last year. Glybera treats a rare disorder that clogs the blood with fat and has been cleared for reimbursement in Germany at a price of 850,000 euros, or around $1 million. It will be sold for a one-time payment because it is too difficult to measure how well it works, said Joern Aldag, CEO at Dutch biotech firm UniQure NV , which developed Glybera. Aldag said gene therapies for larger patient populations in development at UniQure, including hemophilia and congestive heart failure, would likely be priced on an amortized basis. "Gene therapy may be an initial very high burden for the healthcare system," Aldag said. "The general consensus is that asking for a one-off payment of over a million dollars becomes unacceptable." U.S. health insurers and other group payers routinely pay annual prices in the hundreds of thousands of dollars for enzyme replacement therapies for extremely rare, "orphan" diseases. BioMarin's Vimizin costs nearly $400,000 per year for a genetic disorder that causes skeletal malformation and other problems. But insurers are much less willing to foot that kind of bill for drugs used in large numbers of patients. Last year, they pressured Gilead Sciences Inc to cut the $1,000 per-pill cost of its new hepatitis C drug, saying the cost of treating more than 3 million Americans infected with the virus was unacceptable. Gilead said recently that it is discounting its prices by nearly 50 percent. About 20,000 Americans are estimated to have hemophilia, which is caused by a defect in a gene carried on the X chromosome. "Distributing reimbursement over many years recognizes the fact that we don't know how long these (gene therapy treatments) are going to last," said Dr James Wilson, a gene therapy expert at the University of Pennsylvania.
Deze manier van vergoeden is natuurlijk al eerder besproken en biedt het voordeel voor de 'klant', dat ze zeker zijn van een werkende therapie. Bovendien maakt de fabrikant de drempel lager.

Beste IVET, om even kort te gaan, ik denk dat de risico's voor uniQure vergelijkbaar of kleiner zijn. In ieder geval niet groter. Kenmerkend hiervoor vind ik ook bijvoorbeeld de reactie van Aldag op de op zich ook niet onaardige hemofilie data van Baxter. Hij lijkt er niet zo van onder de indruk. Ik vermoed dat Aldag er op vertrouwt dat het eigen hemofilie product (met een AAV-5 vector die al veilig is gebleken in porfyrie en die mogelijk veel geschikter is om de lever te transfecteren dan AAV-8) superieur zal zijn aan dat van de concurrenten. Qure kan in zijn weldra te starten (of misschien inmiddels al begonnen?) studie hoog doseren, met ook hogere factor VII spiegels in de patient tot gevolg. De voorschrijver van de therapie, de arts, zal bij een even veilig bijwerkingen profiel toch snel kiezen voor de effectievere behandeling. Op basis van de breedte van de pijplijn ligt er momenteel een voor Qure een grotere waarde in het verschiet. Ik moet er wel bijzeggen dat een deel van de (preklinische) pijplijn van Spark nog undisclosed is, dus misschien blijkt die binnenkort toch breder te zijn dan we nu denken. Behoudens op het terrein van de hemofilie zijn deze twee bedrijven op dit moment ook niet echt elkaars concurrenten, denk ik, en kunnen ze prima naast elkaar een eigen markt bedienen. Wat erg voor Spark pleit: wellicht al in 2016 een geregistreerd product op de Amerikaanse markt: het eerste! Dat maakt indruk. Daarnaast is Spark een spinoff vanuit de Children's hospital of Philadelphia/university of Pennsylvania: sterke academische basis. Daar kan uniQure toch minder op leunen. Maar goed, Qure heeft zijn tentakels uitgespreid naar verschillende centra binnen Europa. Heidelberg is bijvoorbeeld ook niet de minste op het gebied van hartfalen. Een gelijke waardering komt denk ik vanzelf. Een partnering deal met een US company voor het hartfalen programma zou dit wel eens kunnen katalyseren.

UniQure heeft nog wat aan de weg te timmeren inzake het maken van grote deals (ontlast QURE ook veel meer qua cashburn en geeft de aanwezige middelen de kans om aangewend te worden voor nieuwe targets/trials te onderzoeken/op te starten) Overigens kunnen bepaalde deals dermate groot zijn dat ik me kan voorstellen dat bij de huidige waardering een poging gedaan kan worden om QURE in te lijven. mvg ivet
IVET & Tex, Dank voor vergelijking die denk ik vrij volledig is. Ik denk dat QURE zich pas laat overnemen rond September 2015. De crux in verschil in waardering ligt vlgs. mij in de coverage door grote banken van Spark en niet van Qure. Aldag is een echte heer en geen branie meneer. Zouden ze nu wel wat meer van kunnen gebruiken eerlijkgezegd. Het grote kantelmoment ligt op: 1. Cardio partnering (hopelijk binnekort) 2. Eerste data Duitse patienten hemofilie (Aug/Sep 2015). Als die goed zijn gaan ze flink stijgen. Als niet dan wordt het heel moeilijk. Zelf acht ik de kans op teleurstellende uitkomsten in hemofilie klein, maar niet verwaarloosbaar. Ze hebben positieve data in makaken en dan moet het wel heel raar lopen als die in mensen niet optreden. Spannend zijn de extra hoge doseringen want factor IX levels die op 20% of meer van normaal liggen zijn pas echt ideal vergeleken met St. Judes. Ik denk dat ik helemaal blijf zitten tot deze data publiek zijn gemaakt....

Dat maakt hem inderdaad een heer. Daarbij ben ik ook niet vergeten dat hij eerder keurig rekening hield met de kleine belegger in AMT (daarvan zouden andere bedrijven meer een voorbeeld aan kunnen nemen eerlijk gezegd). Ik ben van mening van Aldag wel branie (en visie) heeft. Getuige de samenwerking met 4D Molecular Therapeutics en de bouw van een eigen productie faciliteit.

Niets aan toe te voegen, behalve dat de overname van InoCard ook van visie getuigt. Knap ook dat dit ook gelukt is ten koste van veel grotere partijen. Verder helemaal mee eens.

Glybera kan in de USA nog steeds gepartnerd worden...
Aldag komt inderdaad heel rustig over, voor sommigen misschien té rustig en sober...maar je moet toch heel wat in je mars hebben om zo rustig onder alles te blijven en je plan te trekken. Een poker&chess-player!

klopt...een klinkende naam kan extra glans geven en een opmaat zijn naar grotere deals voor uitdagende projecten.
