Goed te zien dat Filgotinib in de pipeline bij Gilead t.o.v. ABT-494 bij AbbVie de voorsprong heeft in Crohn's Disease en Ulcerative Colitis (Phase 3 versus Phase 2).www.gilead.com/research/pipeline www.abbvie.com/our-science/pipeline.html Op dit gebied wordt door Gilead de komende tijd met concurrent AbbVie de strijd uitgevochten, misschien door hun voucher in te zetten. Ze moeten snel een nieuwe kaskraker hebben, de aandeelhouders worden ongeduldig. Galapagos blijft hier een bijzondere positie houden als concurrent en partner van AbbVie.

De USA-aanvraag is nog in behandeling. The condition was estimated to be affecting approximately 3.5 in 10,000 persons in the European Union, at the time the application was made.In addition, although satisfactory methods of treatment of the condition have been authorised in the European Union, the sponsor has provided sufficient justification for the assumption that the medicinal product containing 2-((2-ethyl-6-(4-(2-(3-hydroxyazetidin-1-yl)-2-oxoethyl)-piperazin-1-yl)-8-methylimidazo[1,2-alpha]pyridin-3-yl)-(methyl)amino)-4-(4-fluorophenyl)-thiazole-5-carbonitrile will be of significant benefit to those affected by the condition. The sponsor has provided preclinical data showing higher activity on lung fibrosis in comparison to pirfenidone, currently authorised for the condition, and increased antifibrotic activity when used in combination with nintedanib, also authorised for the treatment of the condition . The Committee considered that this constitutes a clinically relevant advantage for the patients affected by idiopathic pulmonary fibrosis. Bij positieve Fase2a resultaten van GLPG1690 in IPF in H2 17 zal de beurskoers volgens Nomura reeds hun koersdoel bereiken.
Goede dag en mooi rapport van Nomura. De CF franchise is door hen gewaardeerd op $30 per aandeel. De geschatte kans op succes van 70% voor CF triple combi is geweldig. Een voorziene miljarden omzet per 2025 betekend een snelle uptake. Wie weet kan er nog een stap (Fase 2B: weken behandeling) worden geskipt danwel gecombineerd met doorgang naar Fase 3, na goede 2a scores Q1 2018. Eerder vatte Maxen dit goed samen middels update Morgan Stanley te oktober. Oh wacht, eerst de goede Fase 1 scores mid 2017. We kijken graag vooruit met een Proteostasis (PTI) wat een fase 3 in vooruitzicht stelt binnen hun investor presentatie (na fase 2a). Ook zij, naast Galapagos, hebben nog veel te bewijzen met de afzonderlijke componenten waarbij hun traject halfjaar achter ligt en triple CF combi anders is vormgegeven (Amplifier i.p.v. C2 corrector). In het licht van aanvraag IND GLPG2222 Galapagos en grotere fase 1 trajecten potentiator GLPG2451 en GLPG2737 (ook IND-Aanvragen H2 2017?), zal het serieuze overweging worden om wereldwijde 'leverage' AbbVie eerder te benutten en snellere development te bewerkstelligen na Fase 2a scores (13-weeks 2B met expansion naar 24/26-weeks Fase 3). De behoefte patiënten is enorm, en anders 2x werving! Een snelle toegang tot CF-patient community USA/ex-USA, en grotere studies aanvangen kan met behulp van grote partner AbbVie.
De twee Fase 3 onderzoeken van Vertex met Orkambi, genaamd TRAFFIC en TRANSPORT, hebben los van voorbereiding (zeg 6 maanden) een 12-maands periode in beslag genomen. Beide studies +500 patiënten. Bij combi Fase 2b/3 kunnen AbbVie/Galapagos medicijn versneld naar de markt brengen, onder mom van positieve vorderingen jaar 2017. Werkzaamheid is preklinisch aantoonbaar resultaat geleverd. Interessante ontwikkeling bij de concurrent Vertex voor CF: hebben CTP-656 rechten gekocht van Concert Pharmaceuticals. 'Vertex plans to develop CTP-656 for potential use in future once-daily regimens in combination with its other pipeline drugs to treat the underlying cause of CF. If CTP-656 is approved for use in combination regimens, Vertex will pay up to an additional $90 million in future regulatory approval milestone payments. Concert is currently evaluating CTP-656 in a phase II study in patients with CF who have gating mutations. Upon closing of the deal, Vertex will be responsible for the candidate’s development. Vertex will also acquire rights to all of Concert's other CF research and preclinical programs, per the deal. With purchase of CTP-656, Vertex will get a potential competitor for its triple combo regimen off its back for a reasonable price of 250 million. Also, CTP-656 - a reformulation of Kalydeco - can potentially be dosed once daily while Kalydeco is a twice daily drug.' 'Meanwhile, the deal should help Vertex maintain dominance in the CF market. Vertex presently markets two CF drugs – Kalydeco (ivacaftor) and Orkambi (lumacaftor-ivacaftor combination). It is also developing many new combination regimens with CFTR modulators. Vertex is evaluating CF corrector VX-661 in combination with ivacaftor. Vertex is also evaluating a couple of next-generation CFTR correctors - VX-152 and VX-440 - as part of a triple combination with VX-661 and ivacaftor. @Opop, ja zeker interessant dat Vertex extern molecuul (potentiator) licenseert. Het feit dat zij CTP-656 rechten kopen, gekend om haar 1-maal daagse dosering als monotherapie wat nu in Fase 2 wordt getest t/m eind 2017 > 20mg/100mg/150mg QD, geeft aan dat dit Vertex dit pad van 1-maal daags triple combi therapie (1 tablet per dag) wil bewandelen, waar Galapagos/AbbVie al eerder voor kozen. Het is een luxe dat Galapagos GLPG2451 als back-up potentiator (ook 1-maal daagse dosis) heeft ontwikkeld en al gebruikt als molecuul binnen dual combi studie met GLPG2222 en per eind april zal starten met triple (GLPG2737 erbij). Bij start begin juli 2017 van Fase 2a hebben Galapagos/AbbVie een grote stap gezet naar verdere opschaling CF-project in grotere studies. Vertex zal met CTP-656 molecuul aan moeten tonen dat het onderdeel van triple kan worden (verdraagzaamheid). Die zekerheid is er niet, gelet ook de milestone-payment ad $90 miljoen hetwelk Concert in toekomst ontvangt wanneer FDA toestaat om CTP-656 op te nemen in klinische studies. Hoe snel Vertex CTP-656 kan doorzetten i.c.m. VX-661/VX-659 of de andere C2 correctors (VX-152 of VX-440 en geziene veiligheidsissues) is af te wachten. In aanvulling op het commentaar op pe26 het volgende. 1. De deal tussen Vertex en Concert Pharmaceuticals betreffende het CF-medicijn met aanduiding CTP-656 overtuigt mij persoonlijk niet erg. Het gaat om het ivacaftor-molecuul, door Vertex ontdekt en gecommercialiseerd en door Concert minimaal gemanipuleerd: één of meer waterstofatomen (H) zijn vervangen door de waterstof-isotoop deuterium (D). Als gevolg hiervan zou de halfwaardetijd bij humane toepassing zodanig worden verlengd dat een eenmaal daagse dosering volstaat om het gewenste therapeutische effect te bereiken. Ivacaftor in de niet-gedeutereerde vorm moet - zoals bekend - om de 12 uur worden toegediend. 2. Het gewijzigde farmacokinetische profiel valt te verklaren uit het gegeven dat deuterium dat zich sterker aan koolstof bindt dan waterstof. Door die sterkere binding worden gedeutereerde moleculen in een lager tempo gemetaboliseerd (afgebroken onder invloed van leverenzymen), terwijl de werking in principe hetzelfde blijft. Chiquer gezegd betekent het dat de farmacokinetiek in gunstige zin verandert, terwijl het farmacodynamisch profiel ongewijzigd blijft (of mogelijk zelfs verbetert door bijvoorbeeld iets lagere toxiciteit). Dat is althans de sterk versimpelde en optimistische theorie. 3. Zoals altijd ligt het allemaal veel gecompliceerder en is het niet met zekerheid te voorspellen of een gedeutereerde variant zich gedraagt zoals gehoopt. Een variabele is bijvoorbeeld, waar en op hoeveel plaatsen in het molecuul H door D wordt vervangen. Het ligt voor de hand dat er alleen al daardoor verschillen in de metabolisatiesnelheid kunnen optreden. Maar er is meer. Het kan zijn dat het de gedeutereerde variant een iets ander metabolisatiepad volgt, waarbij het verbreken van de sterke koolstof-deuterium binding geen rol speelt. Ook kunnen andere metabole omzettingen dan die via de lever (cytochroom P-450) belangrijk zijn. Het farmacodynamisch profiel van de variant kan dus flink verschillen van het oorspronkelijke niet-gedeutereerde molecuul. Het laat zich niet goed voorspellen, ook niet qua toxiciteit, zo weten we nu. Uitgebreide klinische onderzoeken zullen dus de theorie moeten bevestigen. Short-cuts zullen de registratie-autoriteiten niet toestaan. Voor alle duidelijkheid: vooralsnog is nog geen enkel gedeutereerd geneesmiddel tot de markt toegelaten. 4. De technologie om bestaande geneesmiddelen te deutereren wordt al meer dan tien jaar als veelbelovend gezien. Volgens groffe schattingen zou minder dan 10 procent van de door de FDA tot de markt toegelaten geneesmiddelen zich lenen voor zo’n deuterium-switch. Maar dat zijn er hoe dan ook best veel! Onder meer het Israëlische generica-concern Teva heeft zich door de overname van Auspex (in 2015) een positie in deze deelmarkt verworven met het molecuul deutetrabenazine, dat overigens nog steeds de markt moet bereiken. In oktober 2016 zag Teva zich namelijk genoodzaakt om opnieuw de betreffende NDA (New Drug Application) in te dienen bij de Amerikaanse FDA. Dat wijst niet direct op overtuigende kwaliteiten. Dat Teva, groot geworden door geneesmiddelen (oneerbiedig gezegd) na te maken, interesse heeft in deutereren is niet vreemd, want deze technologie kan betekenen dat een geldend octrooi kan wordt omzeild wanneer er geen gedeutereerde varianten in de octrooiaanvraag zijn opgenomen. Overiegens hebben diverse octrooizaken bij de rechter tot uiteenlopende uitspraken geleid. Al met al is de technologie niet echt van de grond gekomen. Concert Pharmaceuticals, waar Vertex de (licentie?)overeenkomst mee heeft gesloten, zet er echter zwaar op in. 5. Kort gezegd komt het er mijn inziens op neer, dat Vertex een gecalculeerde gok neemt om een flink bedrag uit te geven voor de eenmaal daagse toepassing van het bestaande ivacaftor-molecuul. Voor Vertex zou de eenmaal daagse frequentie nodig zijn om qua toedieningsregiem synchroon te kunnen lopen met een of meer andere, eenmaal daags te doseren CF-moleculen die Vertex in ontwikkeling heeft. Of het nieuwe molecuul een toelatingsvergunning van de registratie-autoriteiten krijgt is niet op voorhand zeker. Daarin zit – om te beginnen - de (grote) gok.
6. Er waren alternatieven denkbaar. Vertex had er voor kunnen kiezen om te experimenteren met een moleculaire variant van ivacaftor óf om te sleutelen aan de toedieningsvorm om zodoende de afgifte te vertragen (retard- of depotpreparaat). De eenmalig toe te diene dosis moet dan wel worden verhoogd om gedurende 24 uur de gewenste bloedspiegel te bereiken. Dat betekent dat kort na de inname van de eenmaal daagse dosis, de plasmaconcentratie aanmerkelijk hoger zal moeten zijn dan in het geval twee doses over de dag verspreid worden ingenomen. Hierdoor wordt de kans op een grotere toxiciteit sterk verhoogd. Het eerste alternatief vraagt heel veel (kostbare) inspanningen, het tweede draagt het grote risico in zich, dat men van de regen in de drup komt. 7. Twee snelle conclusies laten zich trekken. Vertex heeft klaarblijkelijk geen ander keus gehad dan ivacaftor te (blijven) gebruiken als basiscomponent (potentiator) in hun zoektocht naar nieuwe combi-therapieën. Voor de absolute marktleider op het gebied van CF-medicatie is dat eigenlijk een zwaktebod. Galapagos heeft een aan ivacaftor klinisch gelijkwaardig molecuul gevonden (de CFTR potentiator GLPG 1837 met eveneens een tweemaal daagse toediening) dat uniek is, ook in die zin dat geen inbreuk op het Vertex-octrooi van ivacaftor wordt gepleegd. 8. Financieel analisten van de investment bank Leerink gaven het volgende commentaar op de deal: "First, most obviously it removes a potential competitor, and, more importantly, a potential combination partner for other entrants who may possess a CFTR corrector. It signals Vertex's willingness to enhance and improve its existing franchise and portfolio in CF, rather than fret over "not-invented-here" considerations. "Second, it offers a potentially improved profile for future multi-drug CF combinations. Third, it can extend the duration of Vertex's CF combinations even longer, given likely composition-of-matter exclusivity for Kalydeco of 2027 in the U.S. and 2025 in Europe, compared to 2032-plus in the U.S. and Europe for CTP-656.” 9. Gelet op de ontwikkelingen in de CF-markt kan ik persoonlijk Concert met zijn CTP-656 niet als potentiële concurrent voor Vertex zien, althans niet als een concurrent die een substantieel marktaandeel zou kunnen afsnoepen. Duo en triple combi’s zullen bovendien naar alle waarschijnlijkheid binnenkort de standaard zijn. Ten tweede: de grootste concurrent van Vertex, Galapagos, heeft al een eigen molecuul dat naar alle waarschijnlijkheid tenminste gelijkwaardig is aan ivacaftor (zie de uitkomsten van het Saphira I fase 2 onderzoek eind 2016). Bovendien heeft Galapagos nog meer pijlen op de boog qua CF-moleculen en doseringsregiems. Er is voor het Belgisch-Nederlandse bedrijf dus geen enkele reden om te pogen het Concertmolecuul te licenseren. Ten derde: het valt zeer te bezien of CTP-656 een beter profiel biedt in de komende Vertex combi-therapieën. Het zou kunnen, maar de stelligheid waarmee Leerink dit oppert, is misplaatst. En tot slot: wat heb aan een quasi-octrooiduur verlenging van ivacaftor als je grootste concurrent op een heel eigen spoor zit met GLPG1837, waar een octrooiconflict helemaal niet aan de orde is? 10. Leerink heeft in mijn ogen klakkeloos opgeschreven wat Vertex heeft verteld. Als dit de argumenten zijn om een deal van een paar honderd millioen dollar (te vermeerderen met uitgaven voor klinisch onderzoek en registratie) met een onzekere uitkomst te onderbouwen, dan vermoed ik dat Vertex de hete adem van Galapagos in de nek voelt, maar weinig opties heeft om te reageren. De deal is in mijn ogen op zijn best een gimmick om de octrooibescherming van ivacaftor te verlengen, maar volkomen ontoereikend om de concurrentie op afstand te zetten.
Respect voor je grote bijdragen, Rekyus. Punt 7 geeft het treffend weer. Werkingsmechanismes Galapagos zijn uniek; dit geld voor GLPG1837 en voor GLPG2451 (de potentiator voor triple). Galapagos kent zijn moleculen en heeft back-ups in ontwikkeling, waarvan is voorzien dat zij in 2017 ook kliniek ingaan. Vertex heeft dit in veel mindere mate danwel overschot aan C2 correctors.

This 20-week Phase 2 study evaluated the efficacy and safety of filgotinib in patients with moderate-to-severely active Crohn’s disease (CD). The primary endpoint (CDAI remission at Week 10) was met with an acceptable safety profile. Here, efficacy data from the exploratory Week 10-20 period as well as overall safety data are presented. Methods: 174 patients with moderate-to-severely active CD (CDAI: 220 to 450) and ulcerations confirmed by centrally read endoscopy were randomized 3:1 to receive 200mg filgotinib (FIL) or placebo (PBO) QD for 10 weeks. Immunosuppressants were to be discontinued prior to treatment initiation but corticosteroid-treated patients remained on stable doses until Week 10. Based on clinical response at Week 10 (CDAI decrease from baseline =100, as calculated by the investigator), patients were assigned to FIL (200mg or 100mg QD) or PBO for an additional 10 weeks. Patients assessed as clinical responders underwent mandatory corticosteroid tapering after Week 10. The second part of the study was exploratory. Results: Baseline characteristics were similar in both initial treatment groups, including mean disease duration (8.3 y), mean CDAI score (293), mean CRP (15.6 mg/L, 41%>10mg/L), oral corticosteroids (51%, mean daily dose 20.8 mg/day). At Week 20, 50–71% of initial FIL 200mg responders showed clinical remission and 67–79% showed clinical response, depending on their assignment to FIL 200mg QD, FIL 100mg QD or PBO. The FIL initial responders also maintained their gains in quality of life, as revealed by an IBDQ score at Week 20 that was at least 38.1 points higher than baseline. 59% (13/22) of patients not responding to PBO after 10 weeks showed clinical response at Week 20 upon being switched to FIL 100mg QD, and 32% (7/22) showed clinical remission. The proportion of patients experiencing at least one TEAE was 75% with FIL (all periods of FIL exposure) and 67% with PBO (all periods of PBO exposure). Serious TEAEs occurred in 9% of FIL patients and 4% of PBO patients. TEAEs leading to discontinuation occurred in 18% for FIL and 9% for PBO. Serious infections were reported in 3% of FIL patients, and none in the PBO group. Conclusion: Clinical efficacy, induced with filgotinib after 10 weeks of treatment, as well as IBDQ improvements were sustained through Week 20 despite mandatory steroid tapering. 100mg filgotinib also showed efficacy, however this needs to be further evaluated. The efficacy and safety data of filgotinib suggest a favourable risk/benefit profile.

This 20-week Phase 2 study evaluated efficacy and safety of filgotinib in patients with active Crohn’s disease (CD). The primary endpoint (CDAI remission at Week 10) was met with an acceptable safety profile. Here, an exploratory subgroup analysis of the first 10 weeks, based upon prior exposure to anti-TNF therapy, is presented. Methods: 174 patients with moderate-to-severely active CD (CDAI: 220 to 450) and ulcerations confirmed by centrally read endoscopy were randomized 3:1 to receive 200mg filgotinib (FIL) or placebo (PBO) QD for 10 weeks. Immunosuppressants were to be discontinued prior to treatment initiation but corticosteroid-treated patients remained on stable doses until Week 10. Patients naïve to anti-TNF therapy as well as patients who were previously exposed to anti-TNF with no response or loss-of-response were included. Endpoints include clinical outcome (CDAI), patient-reported outcomes (PRO: CDAI and IBDQ), histopathology (D’Haens score) and combined clinical-biological response (CDAI and biomarkers). Results: Baseline characteristics were similar in FIL and PBO groups, including mean disease duration (8.3y), mean CDAI score (293), mean CRP (15.6mg/L, 41%>10mg/L), oral corticosteroids (51%, mean daily dose 20.8 mg/day). 42% of the patients were anti-TNF naïve, 58% were anti-TNF non-responder. Clinical remission (CDAI<150) was induced at Week 10 in 47% of FIL patients versus 23% on PBO (p=0.0077). CDAI remission and response were higher in the FIL group versus PBO irrespective of prior anti-TNF therapy. PRO measured by changes from baseline in PRO2 score and general well-being (CDAI component), as well as quality of life assessed by IBDQ improved more in both FIL subgroups compared to PBO. A combined clinical-biological response endpoint confirmed these findings in the subgroup with elevated CRP or faecal calprotectin at baseline. Histopathology at Week 10 showed numerically greater effects after FIL treatment versus PBO for both subgroups (Table 1). FIL was safe and well tolerated. Similar incidences in SAEs, TEAEs leading to discontinuation and infections were observed in both anti-TNF subgroups, with a somewhat higher incidence of TEAEs in anti-TNF non-responders. Conclusion: Efficacy of filgotinib was shown in CD patients independently of their prior anti-TNF exposure, and was consistent across all endpoints. The safety profile was also similar. These data suggest a favourable risk/benefit profile, in both anti-TNF naives and anti-TNF non-responders.
