A second study in the New England Journal of Medicine showed Herceptin can also reduce cancer recurrence when combined with a chemotherapy drug. A leading cancer expert called the studies' findings "stunning". But they will increase pressure on regulators to make Herceptin available for early as well as advanced cancers. Herceptin, which also has the generic name traztuzumab, targets a protein called HER2, which appears to be over-abundant in some women's breast cancers. An estimated fifth of breast cancer cases - around 10,000 women in the UK - are HER2 positive. Roche, which makes Herceptin, has to submit an application to European regulators before the drug can be licensed for use in early-stage disease. The National Institute for Health and Clinical Excellence then has to give its approval for the drug to be prescribed on the NHS. The government has said this process should be fast-tracked. But it is likely to be next spring before Herceptin goes through all these regulatory hoops. The first study, by the Breast Cancer Group of international researchers and sponsored by Roche, looked at the effect of giving Herceptin to breast cancer patients who had already had surgery and a course of chemotherapy. Almost 1,700 women received one year's treatment with Herceptin and a group of equal size were simply observed. After 12 months, 13% (220) of the group under observation had seen a recurrence of their cancer, 34 of whom had died. In the group taking Herceptin, just 127 women (7.5%) had seen a recurrence of their cancer, with 23 cancer-related deaths. Women on a two-year course of Herceptin are currently being followed up by the same researchers. Results so far are also promising. Professor Ian Smith, head of the Royal Marsden Hospital and Institute of Cancer Research, who worked on the study, told the BBC News website: "This is one of the most important developments in breast cancer treatment. It is only a minority of women who will benefit from it, but for that minority it makes an enormous difference." The second study combined results from two US trials covering 3,350 women given Herceptin either in combination with the drug paclitaxel (Taxol) or alone. Other women were given Taxol alone. Taking both drugs together reduced the risk a woman's cancer would return by half, and the risk of death by a third. Dr John Toy, medical director at Cancer Research UK, said: "These are stunning results for women who have got this particular sort of breast cancer. "Each of the trials shows undoubted benefits. These papers are in a top medical journal and the data is robust." But Dr Toy said it would be important to see longer-term results from the studies, as cancers often recurred in HER2 positive women 18 months to two years after their initial diagnosis. Jeremy Hughes, chief executive at Breakthrough Breast Cancer, said the studies showed one of the "biggest breakthroughs in breast cancer treatment."

Het Nederlandse biotechbedrijf Crucell heeft een octrooi verworven op een technologie die de productie een stuk goedkoper moet maken De productie van geneesmiddelen langs biotechnologische weg is een relatief jonge tak van sport. Sinds de voorzichtige start eind jaren zeventig van de vorige eeuw, is er aan het productieproces weinig fundamenteel veranderd. Cellen die zijn afgeleid van de eierstok van Chinese hamsters (CHO, spreek uit: tsjo) zijn de werkpaarden van de industrie. Om deze cellen een therapeutisch eiwit te laten maken dat dient als basis voor een medicijn, wordt een stukje menselijk DNA in de cel gestopt. Het inbrengen van dit transgen leidt echter lang niet altijd tot een kolonie van cellen die het gewenste eiwit produceert, omdat deze productie vaak verloren gaat bij deling van de oorspronkelijke cel. Farmaceutische bedrijven moeten daarom honderden zo niet duizenden kolonies screenen om erachter te komen in welke kolonies het transgen tot uiting komt. 'Dit komt omdat 98% van het DNA uitstaat', vertelt Arie Otte, hoogleraar aan de Universiteit van Amsterdam en medewerker van Crucell . 'Als het transgen terechtkomt in een deel van het erfelijk materiaal dat afgesloten is, wordt het bij de celdeling tot zwijgen gebracht. Alleen als het toevallig belandt in een deel dat openstaat, komt het tot uitdrukking middels de productie van eiwitten.' Het onderdrukken van een belangrijk deel van het erfelijke materiaal is op zich functioneel. Het maakt celdifferentiatie mogelijk en zorgt er bijvoorbeeld voor dat viraal DNA, dat een fors deel van het menselijk genoom beslaat, geen kwaad kan. Maar het feit dat een transgen in de CHO-cel vaak uit gaat of minder goed werkt na celdeling, maakt de biotechnologische productie van medicijnen een extra kostbare aangelegenheid, zegt bestuursvoorzitter Ronald Brus van Crucell . 'Een biotechfabriek om één antistof te produceren kost algauw $ 500 mln. Vandaar dat biotechmedicijnen duur zijn. Sommige geneesmiddelen tegen kanker kosten euro 50.000 per jaar. Als je cellen hebt die meer werkzame stof produceren, gaan de kosten per medicijn omlaag.' Bepaalde eiwitten zijn verantwoordelijk voor het tot zwijgen brengen van genen. Otte deed samen met collega's onderzoek naar het mechanisme dat ertoe leidt dat sommige genen ontsnappen aan de repressie van deze eiwitten. Zij vonden Star-elementen, een nieuwe klasse van kleine genetische elementen die genen vrijwaren van de onderdrukkende werking van de genoemde eiwitten. De onderzoekers hebben over hun bevindingen over repressie gepubliceerd in gerenommeerde wetenschappelijke tijdschriften, onder meer Nature en Science. Op basis van dit wetenschappelijke inzicht ontwikkelde Otte in het bedrijf Chromagenics een manier om Star-elementen te identificeren en te selecteren. Hierop heeft Crucell , dat Chromagenics vorig jaar maart overnam, een octrooi gekregen van het Amerikaans octrooibureau. De octrooiaanvraag voor Europa is nog niet afgerond. De toepassing van Star-elementen in de biotechnologische productie van medicijnen ziet er als volgt uit. Door het transgen dat in de CHO-cellen wordt gestopt te flankeren met schildwachten in de vorm van Star-elementen, komt het transgen binnen het erfelijk materiaal van de cellen vanzelf in een open domein terecht. Daarmee kan de eigenschap van het transgen, de aanmaak van het gewenste eiwit, tot expressie komen. In een laboratoriumopstelling heeft Crucell inmiddels aangetoond dat de Star-technologie een veel hogere voorspelbaarheid oplevert dat een celkolonie de felbegeerde eiwitten produceert dan de traditionele aanpak, vertelt Otte. Bovendien ligt de productie van kolonies die zijn gemaakt met behulp van de Star-technologie gemiddeld een factor vier hoger dan kolonies waarbij deze aanpak achterwege is gebleven. Het eindeloos screenen van geschikte kolonies kan achterwege blijven en de productie per kolonie gaat omhoog, vat Ronald Brus de voordelen van de Star-technologie samen voor een industrie waarin jaarlijks circa $ 35 mrd omgaat. 'De kosten van de omzet gaan omlaag. Daarmee komen medicijnen voor kleinere markten binnen bereik die nu niet worden gemaakt omdat de productie te duur is.' De echte test voor Crucells jongste octrooi komt als de CHO-cellen met het transgen dat voorzien is van Star-elementen, in een roestvrijstalen vat van enkele tientallen kubieke meter worden gestopt om daar na weken pruttelen hun werkzame stof af te scheiden. Een groot voordeel van Star is volgens Brus dat de drempel voor deze test relatief laag is. Crucell gaat er namelijk van uit dat het toevoegen van Star-elementen aan een transgen geen verschil maakt voor de werkzame stof die de celkolonies uiteindelijk produceren - met of zonder Star, de stof is identiek. Dat moet registratie van de verbeterde productiemethode bij de verantwoordelijke autoriteiten een stuk eenvoudiger maken, aldus Brus. Tot nu toe maakt het risico van het niet verkrijgen van een registratie de industrie kopschuw om voor erkende geneesmiddelen de bestaande productieplatforms los te laten - naast de Chinese hamster zijn dat de NSO-cellen, cellen afkomstig van een muis. Met de cellijn Per.C6, gemaakt uit cellen van het netvlies van een menselijke foetus, heeft Crucell een alternatief in huis. Menig farmacie- en biotechbedrijf heeft de cellijn inmiddels in licentie genomen, maar reserveert die voor toekomstige medicijnen of vaccins. 'Door de eenvoudigere registratie kunnen we met Star sneller de huidige markt binnendringen', zegt Brus. 'We zijn bezig te bewijzen dat de stoffen identiek zijn. Dit jaar willen we de "dealflow" voor Star in gang zetten door de technologie in licentie te geven. Drug halves breast cancer relapse rate for some Wed Oct 19, 2005 5:08 PM ET By Gene Emery BOSTON (Reuters) - In studies described as "stunning," researchers reported on Wednesday that a drug already used to treat advanced cancer can prevent half of all breast tumors from reappearing after standard therapy. But the treatment only works in women whose breast tumors carry excessive amounts of a protein known as HER2 that makes the cancer particularly aggressive, and on occasion may cause serious side effects. About one in five women with breast cancer have such tumors, so 42,000 U.S. women could benefit from the treatment. The drug is trastuzumab, sold under the brand name Herceptin by Genentech Inc. in the United States and by Swiss drugmaker Roche in Europe. Approved in 1998 to treat breast tumors that have spread, routine use of the drug cuts the recurrence rate by nearly 50 percent, at least over the short term, two new studies in The New England Journal of Medicine show. "This is a very important finding. It's likely to change the recommended care for patients. But we need to know more about the side effects and the long-term effectiveness," said Richard Gelber of the Dana-Farber Cancer Institute and an author of one study known as the HERA study. "The results are simply stunning," said Gabriel Hortobagyi of the University of Texas M.D. Anderson Cancer Center in Houston, who predicted in a Journal editorial that the findings "will completely alter our approach to the treatment of breast cancer." SIDE EFFECTS But there are potentially troubling side effects. Trastuzumab can damage the heart in some women, although the problems usually fade if the drug is discontinued, Gelber told Reuters. In addition, women who receive the drug after breast cancer surgery are more likely to develop subsequent tumors in the brain or elsewhere in the nervous system. But in an era when doctors are happy if a drug improves the survival rate by just a few percentage points, a medicine that cuts the tumor recurrence rate by nearly 50 percent is considered a significant improvement. Only tamoxifen, a mainstay of cancer treatment, produces such good results, said Hortobagyi. HER2 tumors tend to be resistant to tamoxifen, said Harold Burstein of Dana-Farber. Researchers must still determine if trastuzumab would work even better if given in chemotherapy or for more than a year. In the HERA study, sponsored by Roche, 3,387 women in 39 countries were given standard surgery and chemotherapy but half received trastuzumab every three weeks for a year. A year after treatment ended, doctors found only 127 instances of death, new cases of breast cancer or some other type of tumor among the trastuzumab recipients compared to 220 such cases among volunteers who did not get the drug. About 6 percent of the patients stopped taking the drug because of adverse results. The risk of heart problems was low, the study said, "but this could change with longer follow-up". Results of the second study, actually a combination of two studies partly funded by Genentech, were released in April by the U.S. National Institutes of Health. They found a similar reduction in death or new cancer among women who got trastuzumab while receiving conventional chemotherapy. But the rate of heart problems was much higher -- 4.1 percent against less than 1 percent in the HERA study.

Lekker duffe uitvinding denk je misschien, maar let op: in de Verenigede Staten bestaat Herceptin. Dat is een middel tegen bepaalde vormen van borstkanker. Het geneest niet, maar het verlengt het leven aanzienlijk. Het kost 40.000 dollar. En dat is de reden waarom het in Nederland niet aan patiënten wordt gegeven. Verzekeringsmaatschappijen en ziekenfondsen dekken het niet. Er is geen medicijn heet het dan. Nou, er is wél een medicijn, alleeen is het veel te duur. Het is zo duur omdat het zo verrekte moeilijk is te maken. Hier in Nederland is een productietechnologie om het benodigde eiwit te laten groeien. Die technologie is van ons, die hebben wij bedacht. Bovendien hebben we een manier gevonden om dit medicijn veel goedkoper te produceren zodat het ook beschikbaar kan komen voor Nederlandse vrouwen. Dit soort uitvindingen die allemaal in Nederland zijn gedaan zijn heel belangrijk, maar je hoort er nooit iemand over. De regering roept steeds meer over innovatie. Welnu, dit is pure innovatie, maar soms lijkt het wel of niemand zich ervoor interesseert. Trastuzumab plus Adjuvant Chemotherapy for Operable HER2-Positive Breast Cancer ABSTRACT Background We present the combined results of two trials that compared adjuvant chemotherapy with or without concurrent trastuzumab in women with surgically removed HER2-positive breast cancer. Methods The National Surgical Adjuvant Breast and Bowel Project trial B-31 compared doxorubicin and cyclophosphamide followed by paclitaxel every 3 weeks (group 1) with the same regimen plus 52 weeks of trastuzumab beginning with the first dose of paclitaxel (group 2). The North Central Cancer Treatment Group trial N9831 compared three regimens: doxorubicin and cyclophosphamide followed by weekly paclitaxel (group A), the same regimen followed by 52 weeks of trastuzumab after paclitaxel (group B), and the same regimen plus 52 weeks of trastuzumab initiated concomitantly with paclitaxel (group C). The studies were amended to include a joint analysis comparing groups 1 and A (the control group) with groups 2 and C (the trastuzumab group). Group B was excluded because trastuzumab was not given concurrently with paclitaxel. Results By March 15, 2005, 394 events (recurrent, second primary cancer, or death before recurrence) had been reported, triggering the first scheduled interim analysis. Of these, 133 were in the trastuzumab group and 261 in the control group (hazard ratio, 0.48; P<0.0001). This result crossed the early stopping boundary. The absolute difference in disease-free survival between the trastuzumab group and the control group was 12 percent at three years. Trastuzumab therapy was associated with a 33 percent reduction in the risk of death (P=0.015). The three-year cumulative incidence of class III or IV congestive heart failure or death from cardiac causes in the trastuzumab group was 4.1 percent in trial B-31 and 2.9 percent in trial N9831. Conclusions Trastuzumab combined with paclitaxel after doxorubicin and cyclophosphamide improves outcomes among women with surgically removed HER2-positive breast cancer. Editorial Herceptin and early breast cancer: a moment for caution Published online November 9, 2005 DOI:10.1016/S0140-6736(05)67670-2 DOI:10.1016/S0140-6736(05) 67670-2 It is no surprise that patients with cancer, together with leading cancer charities, are calling for the faster approval of drugs they see as life saving. One UK charity, Cancer Bacup, has identified a “dossier of delay” in current procedures. The chief culprit in Britain is said to be the National Institute for Health and Clinical Excellence (NICE). The story is not so simple. Under considerable political and media pressure, NICE and the Department of Health last week announced the launch of a rapid process for assessing new and potentially life-saving medicines. Five drugs have been targeted for fast-track: trastuzumab (Herceptin), docetaxel, and paclitaxel for breast cancer; rituximab for non-Hodgkin’s lymphoma; and bortezomib for multiple myeloma. While these developments show commendable responsiveness to public concern, they must not be allowed to undermine NICE’s hard-won and well-deserved reputation for independence and scientific rigour. Herceptin is a case in point. In March, 2002, NICE recommended Herceptin for use in women with HER2-positive advanced breast cancer, either alone or in combination with paclitaxel. The process leading to this decision was criticised for taking too long. NICE justified its timing by pointing out the need for careful and thoughtful scrutiny of the available data by an independent advisory committee. NICE’s principal concern was to provide reliable advice. Their spokeswoman noted that, “we felt it right that we allow the independent committee that advises NICE the time it needed to analyse and consider this evidence before they gave us their final advice”. This view is surely correct. Whatever the sense of urgency, it is crucial that NICE resists pressure to make expedient decisions. Three years on, a similar but even more intense debate surrounds the use of Herceptin for early breast cancer. Promising results were initially presented at this year’s American Society of Clinical Oncology annual meeting. An immediate wave of demand for Herceptin grew, despite the fact that the drug was not only unlicensed for this indication but also that its manufacturer, Roche, had not even submitted data for the drug’s approval. Some countries, such as France, bypassed their official approvals procedure to make Herceptin available. In October, these studies were finally reported in the New England Journal of Medicine. In an accompanying editorial, Dr Gabriel Hortobagyi described the results as “simply stunning” and “revolutionary”. He even went so far as to say that Herceptin was “maybe even a cure” for breast cancer. Naturally this comment was picked up and repeated across the world, fuelling demand for rapid access to Herceptin. The excitement is premature. The studies so far reported represent interim efficacy analyses. As Victor Montori and colleagues advised in last week’s JAMA, such analyses may “show implausibly large treatment effects”. They recommend that “clinicians should view the results of such trials with scepticism”. The two NEJM reports use different dosing regimens, making comparisons and conclusions additionally more difficult. It is especially hard to tease apart the results because one of the papers combines results from two trials sponsored by Genentech. Although the “joint analysis was developed and analysed” by both trial teams, it is not made clear whether this synthesis was planned in advance of the start of both trials. The report merely notes that the US FDA and National Cancer Institute approved the joint analysis plan, which may reflect the expectation that neither trial alone would demonstrate a positive result. Comparisons are further hampered by the omission of crucial overall and disease-free survival data, as well as information on cardiotoxicity. However, it is clear that Herceptin can precipitate severe heart failure in some patients. The best that can be said about Herceptin’s efficacy and safety for the treatment of early breast cancer is that the available evidence is insufficient to make reliable judgments. It is profoundly misleading to suggest, even rhetorically, that the published data may be indicative of a cure for breast cancer. Drug regulatory agencies and bodies such as NICE play an important part in translating research evidence into clinical guidance. It is vital that their decisions are made carefully after considering the totality of available evidence. They must be free from political, special interest, or media influence, no matter how well meaning. The debate about the availability of Herceptin to women with early breast cancer demands cooler heads than have so far prevailed, in politics, in public, and even in medical journals. - The Lancet

Row over breast cancer drug grows By Tom Noble Health Editor November 14, 2005 THE row over the effectiveness of a breast cancer drug has increased with Swiss-based drug giant Roche accusing The Lancet, one of the most influential medical journals, of inaccuracy. The unusual offensive yesterday came after The Lancet published an online editorial attacking another prominent journal, the New England Journal of Medicine, over its claims about Roche's breast cancer drug Herceptin. The Lancet said people should be cautious about the trial results and questioned claims in the journal that the drug was "simply stunning", "revolutionary" and "maybe even a cure". The best that could be said about Herceptin was that the available evidence was insufficient to make reliable judgements, The Lancet said. Roche replied that the evidence from the trials was the strongest possible. "Roche stands behind all the data of Herceptin in early breast cancer that are referred to in the New England Journal of Medicine manuscripts," its statement said. "There are a number of inaccuracies that appear within The Lancet article. Many highly respected breast cancer experts from around the world have reviewed and accepted the validity of the impressive data … and assert that they will positively improve treatment options for women." The statement was released by the Sydney office of public relations firm Edelman. Last month Edelman issued a release on behalf of the Australian New Zealand Breast Cancer Trials Group, a body that runs and analyses clinical trials. Group co-ordinator Professor John Forbes said the release and its distribution were paid for by Roche, but Roche had nothing to do with its content. While The Lancet raised important issues, he said, the trial results were outstanding and among the most remarkable he had seen in 30 years. The release said the addition of Herceptin to standard chemotherapy reduced the risk of the disease recurring by 46 per cent. The raw data in the New England Journal of Medicine showed that, after a year, cancer returned in 7.5 per cent of women who took Herceptin (127 of 1694 women) and 13 per cent (220 of 1693 women) who did not take the drug. It said 29 died in the Herceptin group and 37 died who took no Herceptin. In Australia, doctors and medical groups have pushed for Herceptin to be available with government subsidy to women with early-stage breast cancer.
