    Israeli scientists: 'In a few weeks, we will have coronavirus vaccine'
    Once the vaccine is developed, it will take at least 90 days to complete the regulatory process and potentially more to enter the marketplace.

    Israeli scientists are on the cusp of developing the first vaccine against the novel coronavirus, according to Science and Technology Minister Ofir Akunis. If all goes as planned, the vaccine could be ready within a few weeks and available in 90 days, according to a release.
    “Congratulations to MIGAL [The Galilee Research Institute] on this exciting breakthrough,” Akunis said. “I am confident there will be further rapid progress, enabling us to provide a needed response to the grave global COVID-19 threat,” Akunis said, referring to the disease caused by the novel coronavirus.

    For the past four years, a team of MIGAL scientists has been developing a vaccine against infectious bronchitis virus (IBV), which causes a bronchial disease affecting poultry. The effectiveness of the vaccine has been proven in preclinical trials carried out at the Veterinary Institute.
    MIGAL is located in the Galilee.
    “Our basic concept was to develop the technology and not specifically a vaccine for this kind or that kind of virus,” said Dr. Chen Katz, MIGAL’s biotechnology group leader. “The scientific framework for the vaccine is based on a new protein expression vector, which forms and secretes a chimeric soluble protein that delivers the viral antigen into mucosal tissues by self-activated endocytosis, causing the body to form antibodies against the virus.”

    Endocytosis is a cellular process in which substances are brought into a cell by surrounding the material with cell membrane, forming a vesicle containing the ingested material.

    In preclinical trials, the team demonstrated that the oral vaccination induces high levels of specific anti-IBV antibodies, Katz said.
    “Let’s call it pure luck,” he said. “We decided to choose coronavirus as a model for our system just as a proof of concept for our technology.”
    But after scientists sequenced the DNA of the novel coronavirus causing the current worldwide outbreak, the MIGAL researchers examined it and found that the poultry coronavirus has high genetic similarity to the human one, and that it uses the same infection mechanism, which increases the likelihood of achieving an effective human vaccine in a very short period of time, Katz said.
    “All we need to do is adjust the system to the new sequence,” he said. “We are in the middle of this process, and hopefully in a few weeks we will have the vaccine in our hands. Yes, in a few weeks, if it all works, we would have a vaccine to prevent coronavirus.”
    MIGAL would be responsible for developing the new vaccine, but it would then have to go through a regulatory process, including clinical trials and large-scale production, Katz said.
    Akunis said he has instructed his ministry’s director-general to fast-track all approval processes with the goal of bringing the human vaccine to market as quickly as possible.
    “Given the urgent global need for a human coronavirus vaccine, we are doing everything we can to accelerate development,” MIGAL CEO David Zigdon said. The vaccine could “achieve safety approval in 90 days,” he said.
    It will be an oral vaccine, making it particularly accessible to the general public, Zigdon said.
    “We are currently in intensive discussions with potential partners that can help accelerate the in-human trials phase and expedite completion of final-product development and regulatory activities,” he said.

    Moderna Therapeutics, a biotech company based in Cambridge, Mass., has shipped the first batches of its COVID-19 vaccine. The vaccine was created just 42 days after the genetic sequence of the COVID_19 virus, called SARS-CoV-2, was released by Chinese researchers in mid-January. The first vials were sent to the National Institute of Allergy and Infectious Diseases (NIAID), part of the National Institutes of Health (NIH) in Bethesda, MD, which will ready the vaccine for human testing as early as April.

    NIH scientists also began testing an antiviral drug called remdesivir that had been developed for Ebola, on a patient infected with SARS-CoV-2. The trial is the first to test a drug for treating COVID-19, and will be led by a team at the University of Nebraska Medical Center. The first patient to volunteer for the ground-breaking study is a passenger who was brought back to the US after testing positive for the disease aboard the Diamond Princess. Others diagnosed with COVID-19 who have been hospitalized will also be part of the study.

    Remdesivir showed encouraging results among animals infected with two related coronaviruses, one responsible for severe acute respiratory syndrome (SARS) and another for causing Middle East respiratory syndrome (MERS). Volunteers will be randomly assigned to receive either the drug or a placebo intravenously for 10 days, and they will have blood tests and nose and throat swabs taken every two days to track the amount of virus in their bodies. Even if the drug shows some efficacy in keeping blood levels of SARS-CoV-2 from growing, it could help to contain spread of the infection.

    Moderna’s vaccine against COVID-19 was developed in record time because it’s based on a relatively new genetic method that does not require growing huge amounts of virus. Instead, the vaccine is packed with mRNA, the genetic material that comes from DNA and makes proteins. Moderna loads its vaccine with mRNA that codes for the right coronavirus proteins which then get injected into the body. Immune cells in the lymph nodes can process that mRNA and start making the protein in just the right way for other immune cells to recognize and mark them for destruction.

    As Dr. Stephen Hoge, president of Moderna, told TIME earlier this month, “mRNA is really like a software molecule in biology. So our vaccine is like the software program to the body, which then goes and makes the [viral] proteins that can generate an immune response.” That means that this vaccine method can be scaled up quickly, saving critical time when a new disease like COVID-19 emerges and starts infecting tens of thousands of people.

    A biotech company that is one of several developing vaccines and drugs for the novel coronavirus has shipped vials of its investigational vaccine for a U.S. clinical trial.

    Cambridge, Massachusetts-based Moderna said Monday that it had released the first batch of its vaccine against the rapidly spreading COVID-19 virus, mRNA-1273. The vials, for a Phase I clinical trial, have been shipped to the National Institute of Allergy and Infectious Diseases, part of the National Institutes of Health.

    A page for the clinical trial was posted on ClinicalTrials.gov on Tuesday. Sponsored by the NIAID, the safety and immunogenicity study will enroll 45 men and non-pregnant women aged 18-55 and has an estimated start date of March 6, and will recruit healthy volunteers, according to the page. Participants will receive the vaccine at doses of 25, 100 or 250 micrograms. One site – the Kaiser Permanente Washington Health Research Institute – Vaccines and Infectious Diseases, in Seattle – is listed. However, NIAID Director Anthony Fauci told The Wall Street Journal on Monday that the trial would start in April and enroll 20-25 participants, with initial results potentially available in July or August.

    “I want to thank the entire Moderna team for their extraordinary effort in responding to this global health emergency with record speed,” said Juan Andres, Moderna’s chief technical operations and quality officer, said in a statement. “This would not have been possible without our Norwood manufacturing site, which uses leading-edge technology to enable flexible operations and ensure high quality standards are met for clinical-grade material.”

    Moderna is one of several companies that have teamed up with the U.S. government to develop vaccines and therapeutics for COVID-19. Johnson & Johnson, Regeneron Pharmaceuticals and Sanofi are among the others. In addition, a drug developed by Gilead Sciences to treat Ebola virus, remdesivir, is being tested in a clinical trial in China.

    As of Monday, 79,331 people around the world have become infected with the COVID-19 virus, and more than 2,600 have died, the vast majority of them in China, according to the World Health Organization. Since the virus emerged in the central Chinese city of Wuhan in November, there have been new outbreaks in South Korea, Iran and Italy, fueling fears of a global pandemic. However, the WHO has not yet officially declared one. With tens of millions of people in China now on effective lockdown and unable to go to work, there have been rising fears also of supply chain disruption in the drug industry, which sources many of its raw materials from China. On Sunday, Axios reported that the Food and Drug Administration has drawn up a list of about 150 drugs that face shortages if the COVID-19 outbreak worsens.

    Eigenlijk is chloroquine een medicijn dat tientallen jaren geleden werd geïntroduceerd in de bestrijding tegen malaria. Inmiddels wordt het alleen nog gebruikt voor een handjevol reumapatiënten in Nederland. Met zijn bedrijf Ace Pharmaceuticals is Jan Willem Popma op dit moment de enige apotheker in ons land die het middel nog in productie heeft. Ook wereldwijd zijn er nauwelijks meer producenten die het middel verstrekken. "We krijgen vanuit het buitenland vragen naar ons geneesmiddel", zegt de medicijnfabrikant.

    Eerste successen
    Sinds de uitbraak van het coronavirus wordt alles op alles gezet om medicijnen of vaccins te ontwikkelen die het virus kunnen bestrijden. En zo dook ook het middel chloroquine weer op. De eerste testen met het middel lijken hoopvol.

    Chinese onderzoekers gaven het medicijn aan patiënten in verschillende Chinese ziekenhuizen in Hunan, Guangdong en Peking. Zij knapten sneller op dan patiënten die het middel niet kregen. Hun koorts was vaker afgenomen en hun longfunctie meer verbeterd. Wetenschappers van de KU Leuven ontdekten in 2004 al dat chloroquine werkzaam is tegen het SARS-virus, wat net als het huidige COVID-19 ook een variant van het coronavirus is. Het werd toen alleen getest in het laboratorium en niet op mensen, simpelweg omdat er geen SARS-patiënten meer waren.

    Snel en goedkoop te maken

    Volgens Popma is het relatief eenvoudig om zijn productie in korte tijd op te schalen. "In principe kan ik 17 miljoen Nederlanders voorzien van de tabletten. En zelfs produceren voor de rest van Europa", zegt Popma. Chloroquine is namelijk relatief snel en goedkoop te maken. Toch zit er ook voor hem een risico aan vast. "Ik ben een kleine apotheker en kan niet de financiële gok nemen dat ik straks met miljoenen tabletten achterblijf als het coronavirus weer verdwijnt."

    Reactie minister Bruins

    Minister Bruins van Medische Zorg en Sport laat in een reactie aan EenVandaag weten: "Ik vind het belangrijk om alle opties serieus te bekijken. Ik heb deze optie inmiddels voorgelegd aan mijn experts om te onderzoeken wat de mogelijkheden zijn om dit middel in te zetten voor patiënten."

    Malariapreventie
    Chloroquine kan gebruikt worden ter preventie en bij bestrijding bij diverse soorten malaria: Plasmodium vivax, ovale en malariae. In zeer veel gebieden komen inmiddels stammen van chloroquine-resistente Plasmodium falciparum voor. Om die reden worden ook andere antimalaria-verbindingen gebruikt. Met name betreft het dan mefloquine of atovaquone. De combinatie van chloroquine met proguanil kan effectiever zijn bij de behandeling van chloroquine-resistente Plasmodium falciparum dan met chloroquine alleen. Tegenwoordig wordt deze combinatie niet meer door de (Amerikaanse) CDC aanbevolen, omdat er betere combinaties bekend zijn.[2]

    Bijwerkingen
    Bij de dosering die voor de bestrijding van malaria gebruikt wordt zijn de volgende bijwerkingen bekend: maagpijn, jeuk, hoofdpijn en accommodatieverlamming. Bij hogere doseringen worden deze problemen erger. Chloroquinetabletten hebben een onprettige metaalsmaak. Chloroquine mag slechts kortstondig toegediend worden. Bij inname over een langere termijn, maanden of zelfs jaren, kan Chloroquine het zicht erg verminderen, met het risico op blindheid.

    Of deze echt het wondermiddel zijn om het corona virus te bestrijden is de vraag.
