Helemaal mee eens, een geweldig aanpak van een te langzaam verlopend proces! En hoewel vervelend voor de "uitgesloten" vrouwen, voor mannen met RA, AS en PsA is het de enige mogelijkheid om thans in een Filgotinib studie te starten, want met uitzondering van Crohn worden er geen patiënten geworven momenteel. En met de geweldige resultaten in de Finch studies, waardoor de naamsbekendheid van Filgotinib is gegroeid, zullen er genoeg specialisten in de drie genoemde ziekten zijn die hun patiënt willen voorstellen deel te nemen aan de Europese "Manta" studie. fc

En daardoor de kans op een aanzienlijke hoeveelheid omzetderving verkleint.
Wellicht (maar nu speculeer ik) heeft de koersbeweging van vandaag ook te maken met dit nieuws (dat officieel nog geen nieuws is)...

Misschien een heel stomme vraag, maar waarom zou de Manta studie alleen in patiënten gedaan moeten worden? Waarom kan dat niet in gezonde vrijwilligers? Je wil met die studie toch alleen veiligheid en niet werkzaamheid testen?

Misschien moeten we dit interpreteren als 106 in deze studie en 144 in de Manta studie?
Wel degelijk 250 in deze studie, maar 106 daarvan uit EEA-landen (European Economic Area). Hier vind je de EEA-landen:en.wikipedia.org/wiki/European_Econom... De 144 anderen komen uit de non-EEA-landen: Rusland, India, Ukraine, Moldova. In Estonia (wel onderdeel van EEA) zoeken ze 6 patiënten.

In Estonia (wel onderdeel van EEA) zoeken ze 6 patiënten.
Wellicht ten overvloede: De oorspronkelijke Manta UC trial zoals vermeld op clinical trial.gov heeft, m.i. niet toevallig, ook 250 patienten ingepland, onderverdeeld onder diverse landen, waarvan Rusland, Oekraine en India de belangrijkste zijn. Dit moet m.i. niet ge-interpreteerd worden alszijnde dat er EN 250 UC patienten EN 250 RA/AS/PsA worden gezocht. Dat zou namelijk helemaal niet nodig hoeven zijn voor de FDA. De US en EU listing betreffen eigenlijk dezelfde, nu uitgebreide Manta trial. Er worden in totaal 250 patienten gezocht met UC/RA/AS/PsA. Gilead's US listing behelst de centra waar de UC patienten terecht kunnen, Galapapagos' EU listing de centra voor RA/AS/PsA. Het maakt uiteindelijk denk ik weinig uit waar de patienten vandaan komen of welke aandoening ze hebben, als er maar 250 patienten in totaal bij elkaar gevonden kunnen worden.
In de guidelines van de FDA voor tox studies geeft men een getal van 200 patiënten om deze studie uit te voeren. Vraag mij dan ook af waarom het er hier 250 zijn. Zelfs in de eerste versie van de manta studie stond 200 maar werd later verhoogd naar 250.

Je kan natuurlijk berekenen dat de kosten van de extra studie een dusdanige versnelling oplevert dat die kosten terugverdiend worden door verwachte versnelling en dus een veel hogere omzet dan tot nu toe is ingeschat voor filgo.

Galapagos Hedges testicular tox risk with new MANTA 2.0 study Zoals altijd dank voor het plaatsen!... Data could come already next year. In our discussions with European rheumatology KOLs, there was a willingness to support the trial and according to the company, there was a positive reception of the trials in European centers. The completion date is not disclosed, but looking at PSA and AS trials (~130 patients each) that were completed within a 12-15 month timeframe, we could expect results in mid'20, which is already earlier that the original MANTA completion date in 2021. MANTA 2.0 could act as a back up for RA filing. According to Galapagos, the partners could use data from both trials to meet the FDA requirement for testicular tox data. Thus MANTA 2.0 could serve as the back-up for the regulatory filings if the recruitment for MANTA does not pick up the pace. Volgens mij weet Kempen de planning op dit moment ook niet helemaal zeker (en Gil en Gal ook niet). Wel is het natuurlijk positief dat Galapagos bevestigt dat data van Manta1 en 2 gecombineerd kunnen worden voor de FDA. Maar qua tijdspad het lijkt mij met hun niet reeel dat Manta1 of Manta2, of zelfs de combinatie Manta1/2 op tijd afkomt voor Q3 filing bij de FDA. Dus als Galapagos wel die filing in Q3 communiceert, wordt er m.i. voorgesorteerd op een rolling filing, die er (in ieder geval officieel) nog niet is. Als Manta1 wel al op tijd af zou komen om het mee te nemen voor filing in Q3, dan ben ik blij verrast. Een opmerking daarbij is dat deze Manta2 trial in andere indicaties reeds vorig jaar bedacht is. Daarna overleg en goedkeuring van regelgevers, en inplanning van de trial. Daar gaan vele maanden overheen. Dus in ieder geval ergens vorig jaar (Q3, Q4?) is bij Gil en Gal de beslissing gevallen dat Manta2 nodig was, voor versnelling of backup. Nu we een kwart tot half jaar verder zijn zou de situatie natuurlijk anders kunnen zijn, de recruitment voor Manta1 versneld en bijna klaar. Ik ben ieder geval blij met deze Manta2 backup. En welllicht kan Gilead op hun Q1 call op 2 mei al duidelijkheid verschaffen over het Filing/Manta pad voorwaarts.
In its Q3 call, Gilead indicated contemplating to discuss with the FDA the possibility of a rolling submission, i.e. filling MANTA later than clinical RA data (n.b. the submission will still require the full 52w data from FINCHes that is expected May-Jun'19). Considering an upfront request from the FDA for MANTA, foregoing the study for filing completely might not be the best option, however considering rather lenient treatment of FDA approved drugs with preclinical testicular tox, the FDA could be flexible to allow for the rolling submission . Further details will be disclosed on the next Conference Call.

En waarom niet, overigens, gewoon in het PB over de warrant-uitoefeningen meegenomen dat de verkregen aandelen, conform de regels van het warrantuitoefenplan en conform eerdere afspraken, (geheel of gedeeltelijk) zijn verkocht?

Mij komt het eerder over dat manta 1.0 zeer zeer traag gaat.. waarom praat kempen over completion manta 1.0 mid 2020 als galapagos het heeft over Q4 filling filgotinib reuma na uitlezen manta 1.0 in eind Q3. Of is dit de 26 weeks uitlezing en niet de 13 weeks?

ach, een laptop doorstaat de test van testculaire 'toxiciteit' ook niet en toch volop te verkrijgen! Wordt waarschijnlijk in de bijsluiter opgenomen dat er een kans is op onvruchtbaarheid, maar dat niet te meten is of het door ouderdomsverschijnselen of door levensstijl is... Nu kan ik me voorstellen dat heel veel van invloed kan zijn op de vruchtbaarheid, maar dat mensen ook kunnen kiezen voor het één en/of het ander.

Wanneer je een laptop dichtklapt moet je er inderdaad voor zorgen dat je testikels er niet tussen zitten. Wat RA betreft is de tox kwestie alleen al om demografische redenen geen grote issue, maar het is toch het beste, ik neem aan ook om marketingredenen, als dit allemaal naar tevredenheid van de FDA is afgesloten.
